Una cistouretrografía de micción (VCUG en inglés)es una radiografía que se toma a la vejiga y uretra que muestra la capacidad de la vejiga de su hijo, la capacidad de retener y vaciar la orina y si la orina regresa a los riñones (reflujo) durante la micción (acción de orinar). Por qué mi hijo necesita una cistouretrografía de micción? La cistouretrografía de micción se realiza generalmente después de que un niño tiene una infección en la vejiga o los riñones, pero un médico también puede ordenar una cistouretrografía de micción por otras razones. También puede solicitarle una muestra de orina para enviar al laboratorio. Cómo nos registramos para la cita? Llegue 30 minutos antes. Si llega tarde, puede afectar otras citas y retrasar la suya. Si va a llegar tarde o tiene preguntas sobre el examen, comuníquese con Radiología al número Cómo se realiza la cistouretrografía de micción? Para hacer una cistouretrografía de micción se coloca una pequeña sonda blanda y flexible en la vejiga de su niño. No es muy agradable pero tratamos de hacerlo con mucho cuidado y respetando su privacidad. Podría ser de gran ayuda para su hijo que su madre o su padre están presentes para ayudar a sostenerlo. Las niñas se deben acostar en una camilla con las piernas en posición de patas de rana, mientras el tecnólogo limpia la abertura de la uretra con una solución desinfectante. Los niños, con las piernas extendidas. Luego de aplicar un lubricante con lidocaína para entumir, se inserta una pequeña sonda en la uretra que llega hasta la vejiga. Se vacía toda la orina y se conecta la sonda a un envase con material de contraste para radiografías que llena la vejiga de su hijo por gravedad. Por medio de la sonda se le pide que orine, lo que permite ver cómo circula la orina por las vías urinarias. El tecnólogo ubica al niño de costado para tomar radiografías. Un médico observa el proceso con un dispositivo llamado fluoroscopio mientras las imágenes aparecen en un monitor de televisión. Cuando termina el examen se quita la sonda. Cuánto dura el examen? El examen dura aproximadamente 45 minutos. Para poder obtener las imágenes correctamente, su hijo deberá permanecer quieto lo que puede ser difícil para los niños pequeños. Con juguetes, libros y música lo distraemos para que permanezca quieto. La edad del niño nos indicará qué juguetes usar; para los niños menores de 1 año utilizamos correas de seguridad de tela o pequeñas bolsas de arena para inmovilizarlo. También es posible que pidamos que los padres, si lo desean y pueden hacerlo, ayuden a sostener a su niño. Radiología ofrece los servicios de especialistas en vida infantil (Child Life). Son parte del grupo de atención médica y trabajan directamente con su niño y la familia para reducir la ansiedad y mejorar la experiencia en el hospital. Si tiene interés en este servicio, cuando llame para pedir la cita mencione la posibilidad de que un especialista en vida infantil esté presente durante la prueba. Se usa radiación en la cistouretrografía de micción? Sí, su niño será expuesto a una pequeña cantidad de radiación. Entendemos que existe una preocupación especial acerca de la radiación para los niños. Para minimizar la exposición configuramos el estudio de acuerdo a la edad y el peso de su niño además del área a ser examinada. Si tiene preguntas acerca del estudio y si es necesario o no, póngase en contacto con el doctor que lo ordenó. 2 de 4

3 Cistouretrografía de micción Sentirá dolor mi hijo? Puede sentir alguna molestia al insertar la sonda pero haremos todo lo posible para que se relaje y esté tranquilo. El lubricante de lidocaína ayuda a reducir la molestia. Generalmente, en radiología no se administran sedantes para este examen porque puede dificultarle orinar. Su hijo debe orinar durante el examen para que el médico vea el funcionamiento de la vejiga. En casos especiales, podemos usar un sedante si lo solicita el médico que ordenó la prueba. Lo utilizamos para relajar a su hijo y puede también evitar que recuerde lo ocurrido pero no lo duerme. Dado que esta sedación requiere de preparación especial y de un tiempo prolongado de recuperación, se utiliza mayormente con niños que tienen mucho miedo o ansiedad debido a procedimientos anteriores. Si usted siente que su hijo lo necesita, hable con el médico que ordenó el estudio. Qué se debe hacer antes de este procedimiento? Desayuno: no hay necesidad de una dieta especial antes de la cistouretrografía de micción. Puede desayunar normalmente a menos que tenga otro examen (como un pielograma intravenoso) que requiera una dieta especial. Cómo preparo a mi hijo para este examen? 1. Hable con su hijo Explíquele cómo son los pasos del examen en un lenguaje simple y honesto. Primero, estarás acostado sobre una camilla, habrá una máquina de rayos X sobre ti pero no te tocará..." Te pondrán un pequeño tubito en el lugar por donde orinas que sacarán cuando termine el examen." Los médicos y enfermeras pondrán un líquido por el tubo para que puedan ver, en una televisión, cómo está funcionando tu cuerpo. Cuando sientas ganas, puedes orinar." Hable con su hijo sobre lo importante que es su tarea: durante el examen debe quedarse quieto todo el tiempo. Para que esté tranquilo, explíquele que se quedará todo el tiempo presente durante el examen. 2. Relajación Luego de explicarle cómo es el examen, pueden practicar juntos lo que puede hacer si le da miedo y a continuación pueden hablar sobre ese sentimiento. Algunas ideas: respirar profundo, imaginar que está en su lugar favorito o inventar una historia. Algunos niños traen algún objeto que les da seguridad como una cobija o un peluche. 3 de 4

Voy a llevar mi oso de peluche para abrazarlo. Quiero que la persona que haga el examen me explique todo lo que vaya a hacer. Lo que yo tengo que hacer es quedarme quieto como una estatua. Qué ocurre después del examen? Después que limpiemos a su hijo con toallas se puede vestir y retornar a las sus actividades normales. Después de la sonda urinaria, la primera orina puede ser rosada. Esto es porque hay un poquito de sangre en la orina y es normal. Sin embargo, si dura más de 24 horas o si su hijo tiene alguna dificultad para orinar, llame al pediatra. Ofrézcale más líquidos para beber para que vaya más al baño. Esto le ayudará con cualquier sensación de irritación o ardor que tenga luego de la sonda. Otra información importante Avísele al tecnólogo si su hijo tiene alergias, sensibilidad al látex o al colorante material de contraste para radiografías. Recomendamos que los padres acompañen a sus hijos durante el examen pero los hermanos o hermanas no pueden entrar. Téngalo presente cuando haga planes para el día de la prueba. Las mujeres embarazadas tampoco pueden entrar a la sala. Si está embarazada y desea estar con su hijo, hable con anticipación con el tecnólogo para hacer los arreglos correspondientes. Normalmente, un radiólogo lee las imágenes tomadas durante el procedimiento de radiología de su niño en un plazo de 24 horas y envía su reporte al doctor que solicitó el estudio. Si ya tiene una cita programada, su pediatra o un doctor del hospital le explicarán los resultados. Si no es así, llame al pediatra de su niño para pedirlos. 5 Patient and Family Education Voiding Cysto Urethrogram (VCUG) Preparing your child for a bladder test What is a VCUG? A voiding cysto urethrogram (VCUG) is an X-ray of the bladder and urethra. The X-ray pictures taken during a VCUG will show your child s bladder capacity, ability to hold and empty urine and whether the urine moves backward toward the kidneys during urination (reflux). Why does my child need a VCUG? VCUGs are usually done after a bladder or kidney infection, but a doctor can order one for other reasons as well. Your child s doctor may also request that your child give a urine sample so it can be sent to the lab for testing. How do we check in for the appointment? Arrive 30 minutes before your scheduled appointment. Being late can affect other appointments and can delay your appointment. We need to place a small, soft, flexible tube (catheter) in your child s bladder to do a VCUG. This is not very pleasant, but we try to provide great care and privacy. It may help your child to have Mom or Dad there to hold on to. Your child will lie down on the X-ray table. For girls, their legs will be placed in the frog leg position while the technologist cleans the opening of the urethra with a cleansing solution. A boy may keep his legs straight. A numbing lidocaine lubricant may be applied and a small catheter is inserted into the urethra and advanced into the bladder. All urine is drained out. The catheter is then connected to an IV bottle that contains X-ray contrast material. Gravity causes your child s bladder to fill up with the contrast. With the catheter still in place, your child will be asked to pee, which will show how the urine goes through the urinary tract. The technologist will need to position your child on their side for some of the X-rays. A doctor will watch this process with a special device called a fluoroscope while the pictures are shown on a TV monitor. When the test is over, the catheter is removed. How long does the test take? The test takes about 45 minutes. Your child will need to stay still for the exam so that the pictures come out. This can be hard for children. We use distractions, such as toys, books and music, to help your child stay still. The age of your child helps us plan what tools to use. For children under the age of 1, we often use cloth safety straps or small sand bags to keep them from moving. Parents who are willing and able may be asked to help hold their child as well. Radiology offers Child Life services. Child Life Specialists are members of the healthcare team who work directly with your child and family to help reduce anxiety and adjust to the hospital experience. When you call to schedule the exam, tell the scheduler if you are interested in having a Child Life Specialist. Does the VCUG use radiation? Yes, your child will be exposed to a small amount of radiation. We understand that radiation dose is a special concern for children. To minimize how much radiation your child is exposed to, we customize the x-rays based on your child s age, weight and area to be examined. If you have questions about the exam and whether it is needed, please contact the doctor who ordered the VCUG for your child. Will my child have any pain? There may be some discomfort when the catheter is put in. We will do all we can to help your child relax and feel more comfortable. The lidocaine lubricant will help reduce the discomfort. 2 of 4

7 Voiding Cysto Urethrogram (VCUG) Radiology does not normally give sedation for this exam. This is because the medicine can make it hard for your child to pee. Your child needs to pee during the test so that the doctor can see how your child s bladder works. In special cases, we can use sedation if requested by the referring doctor. The sedation we use relaxes your child and may help them not to remember the procedure, but it does not put them to sleep. Because this sedation requires special preparation and prolonged recovery time, it is mostly used with kids who have a lot of fear or anxiety about past procedures. If you feel your child needs this sedation, please talk to your primary care (referring) doctor. What do I do before this procedure? Breakfast: No special diet is required before the VCUG test. Give your child breakfast as usual, unless another exam (like an IVP) is also being done that requires a special diet. How do I prepare my child for this test? 1. Talk to your child Talk about the steps of the test in simple, honest language. First, you ll be lying down on the X-ray table. Above you will be the X-ray machine. The machine is above you but does not touch you They will put a small tube into the place where you pee, and they will take the tube out when the test is over. The doctors and nurses will put some clear liquid into the tube so that they can see how your body is working on the TV monitor. When you feel like you have to go to the bathroom, you can pee. Talk to your child about the important job of holding still for the test. Reassure your child that you will be with them during the test. 2. Practice relaxing After talking about the steps of the test, practice things your child can do if they feel worried or afraid. Discuss these feelings with your child. Some ideas are: taking deep breaths, imagining a favorite place or making up a story. Some children bring a security item, such as a blanket or stuffed animal, with them. 3. Make a written plan together Write down the plan with your child. This will help your child feel a sense of control. An example of the plan might be: Tyler s plan I want my mom or dad right next to me. I am going to bring my stuffed bear to hold. I would like the person doing the test to tell me what the steps are. My job is to hold myself still like a statue. 3 of 4

From outside the hospital, call the toll-free Family Interpreting Line Tell the interpreter the name or extension you need. What happens after the test? We will clean your child with washcloths and towels, and then they can get redressed. Your child may return to normal daily activities. After having a urinary catheter, your child s urine may look pink the first time they urinate. This is caused by a small amount of blood in the urine. This is normal, but if it lasts longer than 24 hours, or if your child has a hard time urinating, call your child s primary care doctor. Give your child extra fluids to drink so that they go to the bathroom more often. This will help with any irritation or burning sensations that can happen after having a catheter. Other important information Please tell the technologist if your child has any allergies or sensitivities to latex products or X-ray contrast material. Although we encourage parents to be with their child during the test, siblings are not allowed in the exam room. Please keep this in mind when you plan your visit. Pregnant women cannot be in the exam room. If you are pregnant and wanted to be with your child, discuss an alternative plan with the technologist ahead of time. The images from your child s radiology procedure are typically reviewed by a radiologist within 24 hours and a report will be sent to the doctor who ordered the test. If you have a clinic or doctor appointment scheduled, the doctor will review the results with you then. If you do not have a visit scheduled, you should call your child s doctor for the results.
