PGF
PGF
WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
HISTORIA Término acuñado por Nevin en 1938. OMS : Tumor neuroepitelial ( glial) de origen desconocido: Gliomatosis cerebri. Astroblastoma. Glioma cordoide del III ventrículo. WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
EPIDEMIOLOGÍA Cualquier edad. Pico 40-50 años.? =?, con presentación más temprana en varones. Taillibert S, Chodkiewicz C, Laigle-Donadey F, Napolitano M, Cartalat- Carel S, Sanson M. Gliomatosis cerebri: a review of 296 cases from the ANOCEF database and the literature.
LOCALIZACIÓN Patrón de crecimiento difuso. Áreas más frecuentemente afectadas: Hemisferios cerebrales : 76% Mesencéfalo : 52% Tálamo : 43% G.Basales : 34% Cerebelo : 29% Leptomeninges: 17% Médula espinal: 9%. WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
MANIFESTACIONES CLÍNICAS Déficits en tractos córticoespinales (58%). Demencia ( 44%). Cefalea ( 39%). Alteración pares craneales ( 37%) HTI ( 34%). Alteraciones nivel conciencia ( 20%). Alteraciones del comportamiento ( 19%). WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
T1: imagen hipo-iso intensa. T2: imagen hiperintensa Yu A, Li K, li H. Value of diagnosis and differential diagnosis of MRI and MR spectroscopy in gliomatosis cerebri. Eur J Radiol. 2006 Aug; 59(2):216-21
Diagnóstico antemortem ( RNM + Biopsia) : difícil. OMS: confirmación diagnóstica requiere estudio postmortem. WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
Puede desarrollarse a partir de tipo I WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
Núcleos ovalados o fusiformes, a menudo hipercromáticos. Actividad mitótica variable. Habitualmente no proliferación microvascular. WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
INMUNOHISTOQUÍMICA S100 y PGF : Negatividad no excluye diagnóstico. WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
HISTOGÉNESIS Controversia sobre célula de origen. Preferible considerarla neoplasia de origen desconocido. WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
WHO: Pathology and Genetics of Tumours of the Nervous System. Kleihues P and Cavenee WK. 2000
