Phenotypes of airway disease COPD and asthma patients with FAO both possess incompletely reversible airflow obstruction. However, the majority of asthma patients display airflow obstruction that remains completely reversible.
Immune cells involved in COPD Leucocytes, including macrophages, neutrophils and T-cells, are recruited to the lung of COPD patients via the actions of various chemokines. Once activated, they contribute to the characteristic emphysema, mucus hypersecretion and chronic bronchitis of COPD via the production of effector molecules, such as cytotoxic mediators,enzymes and Th1 cytokines. Fenotipos clínicos La heterogeneidad de la EPOC impide su descripción exclusivamente basada en el FEV1. La denominación de fenotipo se utiliza para referirse a las formas clínicas de los pacientes con EPOC. El fenotipo debería ser capaz de clasificar a los pacientes en subgrupos con valor pronósticoque permitan determinar el tratamiento conmejores resultados clínicos. Han MK. Clasificación de gravedad de la EPOC: puntos clave La clasificación de gravedad de la EPOC debe hacerse de forma multidimensional(bode/bodex). Se establecen 5 niveles de gravedad pronósticade la EPOC: I (leve), II (moderada), III (grave), IV (muy grave) y V (final de vida). La gravedad de la obstrucción se clasifica según los 4 grados de GOLD: leve (FEV 1 >80%); moderada (FEV 1 :50-80%); grave (FEV 1 :30-49%); muy grave (FEV 1 <30%). Otros parámetros como la puntuación CAT (COPD assesment test), la intensidad de los síntomas o el número y gravedad de las agudizaciones también deben valorarse en la toma de decisiones terapéuticas.

Adecuación del tratamiento durante el seguimiento: fenotipo mixto Se puede ensayar La reducción de dosis de corticoides inhalados hasta encontrar la dosis mínima eficaz. No se recomienda dejar a los pacientes con fenotipo mixto EPOC-asma con tratamiento con broncodilatadores de larga duración sin corticoides inhalados. Aunque se debe individualizar la indicación, no se debe intentar retirar los CIs a los pacientes con prueba broncodilatadora positiva o con eosinofilia en esputo durante el tratamiento con CIs. Definitions Asthma Asthma is a heterogeneous disease, usually characterized by chronic airway inflammation. It is defined by the history of respiratory symptoms such as wheeze, shortness of breath, chest tightness and cough that vary over time and in intensity, together with variable expiratory airflow limitation. [GINA 2014] COPD COPD is a common preventable and treatable disease, characterized by persistent airflow limitation that is usually progressive and associated with enhanced chronic inflammatory responses in the airways and the lungs to noxious particles or gases. Exacerbations and comorbidities contribute to the overall severity in individual patients. [GOLD 2014] Asthma-COPD overlap syndrome (ACOS) [a description] Asthma-COPD overlap syndrome (ACOS) is characterized by persistent airflow limitation with several features usually associated with asthma and several features usually associated with COPD.
