MENSAJES FINALES Los nuevas opciones terapéuticas son eficaces en mejorar el control metabólico, reduciendo el riesgo de hipoglucemia y facilitando el control ponderal. Los objetivos terapéuticos deben ser combinados contemplando no solo los valores de HbA1c, sino también otros factores que contribuyen al desarrollo de complicaciones. Las nuevas combinaciones de tratamientos incretínicos e inhibidores de SGLT-2 ofrecen nuevas perspectivas eficaces, integrales y seguras en el segundo y tercer escalón de tratamiento tras fallo o intolerancia a metformina. Las nuevas insulinas basales ofrecen menor riesgo de hipoglucemia y variabilidad glucémica atenuada, facilitando la optimización especialmente en pacientes con inestabilidad metabólica. Los resultados iniciales avalan la eficacia y seguridad de las combinaciones de insulina basal con idpp4, isglt-2 y agonistas de GLP-1. Los efectos integrales de las nuevos tratamientos sobre la morbimortalidad cardiovascular junto a sus posibles mecanismos pleiotrópicos abren una nueva etapa en el abordaje terapéutico de la diabetes.
