My name is Diane Bodurka. I am the Vice President for Medical Education and Professor of Gynecologic Oncology at The University of Texas MD Anderson Cancer Center. The objectives of this presentation are the following: Number one, to define various endpoints for clinical cancer trials. Next, distinguish between Phase 0, I, II, III, and IV trials. And finally, to identify the ethical principles which are specific to clinical research. 2 First, we re going to talk about what can --- constitutes a clinical trial. It is methodical investigation of the outcome of an intervention used to treat a disease or clinical condition in patients. Because we are treating patients, it is important that the conduction of this trial uses rigorous methodology and design as well as clinical expertise to monitor treatment outcomes of our patients. Finally, it is key that ethical principles must be strictly observed. Primero hablaremos sobre qué es un ensayo clínico. Es fundamental respetar los principios éticos. So why do we do clinical trials? First, to determine the safety of an available therapy, to identify effective therapies in diseases where no effective therapy is currently available; to improve the effectiveness of available therapy; to reduce the toxicity of therapy while maintaining known effectiveness of treatment and this involves quality of life issues; and finally to expand the application of a treatment strategy known to be active in one category of disease. 3 The endpoints of clinical trials have been defined in the literature. We know that the endpoint must be prospectively defined within the protocol. We look at the response to treatment which can include complete response, partial response, or biomarker response, such as a drop in the CA-125 for patients with ovarian cancer. We look at the duration of the response, how long the patient either has a decrease in the amount of disease or regresses to no disease, the time to disease progression, side effects of treatment, as I said before, quality of life, and now in this economic environment there is more --- there is more scrutiny paid to resources and we are lis --- looking at cost effectiveness. Finally, and obviously, we look at overall survival. It is important to define efficacy and toxicity in clinical trials. We use the RECIST criteria to look at tumor response and this is response criteria in solid tumors. We look at improved, progression-free or overall survival which are based on the results of randomized trials, and finally, we utilize standardized criteria to assess side effects. The criteria have been developed by the National Cancer Institute Common Terminology Criteria for Adverse Events. 4 So how do we measure endpoints? We can look at endpoints by looking at physical exam. So if a patient has a skin lesion we measure that with calipers. We look at imaging studies, such as CAT scans and MRIs, laboratory tests including biomarkers, toxicity questionnaires to see how the treatment is affecting our patients, quality of life scales, and again, financial analyses and life projection analysis. Study design is critical to the trial so that one day it may be published. We need to have a hypothesis that is clearly stated. The study needs to have clear scientific and clinical background. We use a detailed treatment outline which is well detailed in the study. There are tests and follow-up criteria that are specified as well as the time that these tests are obtained. There is always a clause for modifications for observed side effects. So if the trial is too toxic a lot of times we have a way to decrease the dose. The end points and the population must be well described. And finally, it is critical that the statistical design fits the hypothesis and the determined end points. 5 So there are four types of cancer trials. First is Phase 0 and in this trial really participation does not help the volunteer but it may help cancer patients in the future or other patients. So Phase 0 studies are exploratory studies that use a fe --- [excuse me] a few small doses of a new drug in each patient. We test to determine if the drug reaches the tumor, how the drug acts in the human body, and how cancer cells in the human body respond to the drug. The patient may need extra biopsies, scans, and blood samples as part of the study. But again, because only a few small doses are tested in each patient, participation does not help the volunteer. Phase I trials focus on safety, pharmacokinetics, and defining the optimal dosing and schedule of the drugs. These studies obtain preliminary evidence for biological activity of an agent or combination of agents and they explore the utility of biomarkers. The key thing to remember about Phase I studies is that they only look at toxicity. So when patients are enrolled in Phase I studies the primary endpoint is toxicity rather than response to the regimen. Phase 2 studies define the level of biological activity which is generally called the objective response rate in a specific setting, in a particular tumor type, with a defined dose and schedule of antineoplastic agents. So again, the RECIST criteria that I talked about earlier are used in the Phase 2 as well as the Phase 3 studies. Phase 3 is really the gold standard. It is the randomized comparison of an experimental regimen to a standard option. There are variable end points including overall survival, progressionfree survival, response rate, reduced toxicity, and improved quality of life. So a good example of this is in the ovarian cancer population the gold standard is Taxol and carboplatin. In this Phase 3 study you can combine an additional agent such as bevacizumab and the two arms are Taxol and carboplatin versus taxol-carbo and the bevacizumab. The studies --- The patients are randomized to one of either treatment groups and then we look at the defined end points. Existen cuatro tipos de ensayos de cáncer. Por ejemplo, en la población con cáncer de ovario, el parámetro por excelencia es el Taxol y el carboplatino. En un estudio de fase 3 se puede combinar un fármaco adicional, como bevacizumab, en cuyo caso los dos subgrupos son Taxol y carboplatino, comparado con Taxol y carboplatino más bevacizumab. 6 Phase 4 studies are a post-marketing evaluation of the efficacy and/or toxicity in the general population. So these have already been approved by the FDA and these drugs are in use. The purpose is to determine if a specific drug or a therapy is safe over time or to see if the drug or therapy can be used in clini --- other clinical situations. So in the past we have known that a drug is --- [excuse me] is successful in treating patients with a low-grade stomach cancer we started using the same drug in patients with ovarian cancer and we ve seen a good response. aleatoriamente a uno de los grupos de tratamiento y luego observamos los criterios de valoración definidos. So what are the necessary elements for a trial? Obviously, we need patients. We need competent clinical investigators, research nurses, data managers, and pharmacists. There has to be significant and efficient data processing as well as secure storage for the data, biostatistical expertise, administrative support, clinical and laboratory support facilities. It has to be approved through the Institutional Review Board. And finally there is always a Data Safety Monitoring Board, which looks at safety at specific endpoints in the trial. 7 The ethical principles of clinical research are four. First, autonomy, the patient must freely elect to participate in the trial. Next is beneficence, that there is a potential benefit associated with participation. Non-maleficence, risk of possible harm from participation must be acceptable based on the realistic potential for benefit. And finally, justice, the conduct of the trial must be fair and not exclude a patient population based on non-clinical considerations, such as age or ethnicity. In terms of the Institutional Review Board, this board is responsible for ensuring the ethical conduct of human subjects research. It is important that the informed consent document states in clear language, which is written at the 8th grade level, the purpose of the study, how the study is being performed, potential risk from participation, and finally available alternative options. The document also informs patients of their rights, who they may contact for concerns related to the study or their treatment program. And actually the name of the principle investigator and their phone number is also included so that the patients may access that individual if they have questions. 8 Patients must provide written consent. Language assistance and translation are available as needed. And if a minor participates, the minor must assent to the trial and there must be informed consent by the adult. Los pacientes deben dar su consentimiento por escrito. So how do we report the results of these trials? 9 So who conducts cancer clinical trials? It can be a - -- a single institution where either a single investigator or multiple investigators work together. It can be done at several institutions where here the efforts are focused on one single clinical trial where the --- all of the institutions collaborate, not-for-profit cooperative groups, such as NCI-funded or CCOP and finally pharmaceutical, biotech multiinstitutional, and now increasingly multinational efforts. So in summary, clinical trials are essential in identifying effective therapies and expanding the application of treatment strategies for patients with various cancers. Prospectively defined clinical endpoints may include response, time to progression, survival, and quality of life factors. And please remember that patients must be free to elect to participate in clinical trials and understand the potential risks and available alternatives.
