1 Scope Objetivo This procedure contains Deep South s requirements for entering spaces that meet the requirements of OSHA s confined space regulation. Background OSHA s definition for a confined space is as follows: "Confined space" means a space that: (1) Is large enough and so configured that an employee can bodily enter and perform assigned work; and (2) Has limited or restricted means for entry or exit (for example, tanks, vessels, silos, storage bins, hoppers, vaults, and pits are spaces that may have limited means of entry.); and (3) Is not designed for continuous employee occupancy. Entry into these confined spaces may present a variety of hazards such as asphyxiation, chemical exposures, electrocution, entrapment, and injury from mechanical energy. Careful control of entry into confined spaces is necessary and required by OSHA No person may enter a confined space until the space has been rendered safe for entry. Confined spaces identified at Deep South workplaces have been identified as "Non-permit Required Confined Spaces" according to the following definition from the OSHA standard: "Non-permit confined space" means a confined space that does not contain or, with respect to atmospheric hazards, have the potential to contain any hazard capable of causing death or serious physical harm. Application Aplicación We have four identified situations that meet the confined space definition, as follows: (1) Work, during construction, on the inside of the VersaCrane outriggers after they are closed up, or are in the process of being closed up, on one end. The work inside the outrigger and having only one exit point meets the OSHA requirements for a confined space. There are limited hazards inside the outriggers during the last phase of construction. These hazards consist of nuisance levels of welding fumes and dye-penetrant weld-testing, hazards from grinding, and heat from the welding preheaters. (2) Construction of the VersaCrane bowls. Welding inside the bowl can pose the same hazards as those noted in (1) above. (3) Maintenance on VersaCrane bowls in the field. Hazards in this application are again related to those posed by welding as noted in (1) above. Also, on client facility sites, additional hazards possibly could include a facility gas release/gas buildup inside the VersaCrane bowl, and the possibility of a facility emergency requiring site evacuation. (4) Entry into large boom sections for the purpose of mechanical repairs. Hazards in this case include those posed by welding inside the boom, heat levels inside (particularly in summer), and the restricted space inside. It is our intent to meet the requirements of a Non-permit confined space" defined as: A confined space that does not contain or, with respect to atmospheric hazards, have the potential to contain any hazard capable of causing death or serious physical harm. Any welding of exotic metals, use of toxic materials, or introduction of other hazardous materials requires a review by the Safety Department in advance of the work. The specific issues and control methods will be defined in the JSA and reviewed with all affected employees. Entrants and attendees have attended the Deep South safety meeting on Confined Space Entry. Review necessity for entry into a confined space and use alternative means of accomplishing the work without entry if possible. Other workers in the area are informed the work is going on inside the confined space. Clients, who may have additional requirements, have been notified if work is being done on their property. The Man Working Inside sign is posted near the entrance to the space. Periodically check on the progress of the work inside the crane component and assure all requirements of this procedure are being met. Reviewing the scope of the work to be performed inside the crane component and identifying hazard and resolution considerations by completing a JSA. Posting a Man Working Inside sign at the entrance to the space. Wearing PPE to protect against any hazards which may be created by the work, including dust from grinding or welding, hearing protection (double may be necessary for such work as grinding), respirators, etc. Keeping out of the confined space any flammable or toxic chemicals unless necessary for the work, and then only in the smallest quantities practical. A source of supplied air may be necessary to sweep welding fumes, dye penetrant vapors, or cutting torch gases from the interior of the confined space. Shutting off oxygen and fuel at the cylinders when not in use and removing the oxy-fuel torch and hoses from the confined space during breaks. Always keeping gas cylinders located out of the confined space. Always checking gas hoses for leaks prior to use. Being alert for any indication the confined space is affecting them (slurred speech, nausea, incoherence, light headedness or dizziness) and exiting or notifying the attendant. Assuring any AC powered tools used are used with a functional GFCI. Exiting the confined space at any time conditions do not seem right or if concerns develop. R Asegurarse que la persona que se encuentre adentro sea informada de cualquier situación que pueda afectarle. Work inside the identified Deep South confined spaces pose the following hazards: 1. Heat from welding pre-heater burners applied to the outside of the outriggers and bowls during construction. Welders have to enter and forced ventilation and an attendant are required throughout the entry period. 2. Gases/fumes if a cutting torch or welder has to be used. The torch must be removed from the confined space during breaks and ventilation with an air horn must be used to ventilate the interior. 3. Entry into the VersaCrane bowl for maintenance any time the crane is assembled and therefore subject to potential movement LOTO techniques must be used in this case. 4. Any work in these spaces on client property may pose the additional hazards of client gas release/gas buildup/client facility emergency. Attendants must be cognizant of these possibilities and be prepared to notify the entrant to exit the space. 5. Entry into large hydraulic crane boom sections for maintenance poses the hazard of heat levels becoming elevated inside the boom. These heat levels must be controlled by scheduling the work when the boom is not in the direct sun in summer. If work must be done in hot conditions, heat stress control measures such as additional water consumption, forced ventilation, and regular breaks must be used in accordance with the Deep South Heat Stress Prevention procedure. 6. The hazard of potential boom movement must be mitigated by proper use of LOTO. and getting trapped are hazards that must be managed Confined Space Procedure Rev. 06/2011 Page 5
