Qué es el yodo radioactivo (I-123)? El yodo radioactivo I-123 es una substancia que se utiliza para ver dónde se encuentra algún tumor tiroideo en el cuerpo. Tanto el cáncer de tiroides como el tejido tiroideo atrapan el yodo. Éste nos ayuda a localizar los lugares donde está activo el cáncer de tiroides. Este procedimiento se denomina estudio diagnóstico del cáncer de tiroides. Si se encuentra cáncer de tiroides durante el estudio, es posible que se le tenga que tratar con una dosis de yodo radioactivo llamado I-131. Esto se denomina dosis terapéutica. Cómo actúa el yodo radioactivo? Le pediremos que trague una pequeña dosis diagnóstica de I-123. Sus intestinos lo absorberán rápidamente y entrará a su corriente sanguínea. Luego se dirigirá a su tiroides y a cualquier sitio de cáncer de tiroides. La mayor parte del I-123 que su tejido tiroideo no absorba se eliminará de su cuerpo por medio de la orina y las heces. El yodo I-123 emite rayos gama (similares a los rayos X), los cuales se pueden ver con cámaras especiales para Medicina Nuclear. Usted tragará una pequeña cápsula que contiene una dosis de diagnóstico (5 milicurios) de I-123. Al día siguiente, utilizaremos cámaras de Medicina Nuclear para tomar imágenes de su cabeza, cuello y cuerpo para buscar tejido tiroideo y sitios de cáncer de tiroides. 2 Cuáles son los efectos colaterales del examen? Los efectos colaterales del I-123 son muy raros. Sin embargo, existe una ligera probabilidad de que usted pudiera tener algo de náusea el día que trague la cápsula. Cómo prepararse para el examen Dieta Usted tiene que estar bajo una dieta con contenido bajo de yodo durante 1 semana antes de su estudio de cáncer de tiroides. Medicamentos Si usted es paciente de withdrawal, tendrá que dejar de tomar: Synthroid (Levothyroxine). Deje de tomarlo durante al menos 6 semanas antes de su estudio. Cytomel (Liothyronine). Deje de tomarlo durante 3 semanas antes de su examen. Si usted es paciente de Thyrogen rtsh, no tiene que dejar de tomar ninguno de sus medicamentos para la tiroides. Usted recibirá inyecciones de Thyrogen rtsh en 2 días diferentes antes de tomar la cápsula de I-123. El día que tome la dosis de diagnóstico de I-123 No coma ningún alimento sólido ni beba nada más que agua durante 2 horas antes y después de su cita. Le alentamos a beber agua. Se le extraerá sangre para las pruebas de laboratorio antes de recibir la dosis de diagnóstico. Estas pruebas controlarán su recuento sanguíneo y los niveles de hormona tiroidea. Para las mujeres Usted tiene que informarnos si está embarazada. Si va a recibir terapia con I-131 después de su estudio de cáncer de tiroides, tiene que someterse a una prueba de embarazo antes de tomar el yodo radioactivo. Esta prueba se hará nuevamente el día que tome la dosis de I-123. Podría volverse a hacer el día de la terapia con I-131. Por favor, infórmenos si está amamantando. Las mujeres que están amamantando no pueden recibir yodo radioactivo, ya que se lo podrían pasar al niño a través de la leche materna. Las mujeres que se someterán a terapia con I-131 después del estudio de cáncer de tiroides por lo común tienen que dejar de amamantar durante 3 meses después de recibir la dosis de I-131. 3 Después de tomar la dosis de diagnóstico de I-123 No coma ningún alimento sólido ni beba nada más que agua durante 2 horas después de tragar la cápsula de yodo. Un estómago vacío asegurará que el cuerpo absorba el suficiente yodo radioactivo. A partir de 2 horas después de la dosis, le alentamos a seguir bebiendo mucha agua. También puede beber jugos. Recuerde, tendrá que volver a Medicina Nuclear para una tomografía al día siguiente de tomar la dosis de diagnóstico de I-123. Permanezca bajo una dieta con contenido bajo de yodo mientras se esté sometiendo a las exploraciones de diagnóstico y a cualquier terapia con yodo. La noche antes de la exploración Use un laxante sin receta médica (uno que puede comprar sin receta) la noche anterior a la exploración. Asegúrese de usar uno que no contenga yodo. Algunos laxantes sin yodo son Miralax, Bisacodyl, Leche de Magnesia, Citrato de Magnesio y supositorios de Ducolax. Al tomar una laxante para vaciar el intestino se elimina cualquier I-123 adicional que se haya acumulado en las heces. Las heces que contengan I-123 podrían dar una lectura positiva falsa (incorrecta) durante la exploración. Después de la exploración Si usted NO se va a someter a terapia con I-131, puede reanudar su dieta normal después de la exploración final. Si usted se VA a someter a terapia con I-131, tiene que esperar para reanudar su dieta normal durante 3 días después de tragar la cápsula de I-131. Precauciones de seguridad contra la radiación No hay ninguna precaución especial que se exija después de tragar la dosis de I-123. 5 UW MEDICINE PATIENT EDUCATION Thyroid Cancer Survey Using I123 Information, instructions, and precautions This handout gives instructions for patients who will receive radioactive iodine to help look for thyroid cancer. What is radioactive iodine (I123)? Radioactive iodine I123 is a substance that is used to see where any thyroid tumors are in your body. Both thyroid cancer and thyroid tissue trap iodine. This helps us find where there is active thyroid cancer. This procedure is called a diagnostic thyroid cancer survey. If thyroid cancer is found during your survey, you may have it treated with a dose of a radioactive iodine called I131. This is called a therapeutic dose. How does radioactive iodine work? We will ask you to swallow a small diagnostic dose of I123. Your intestines will absorb it quickly, and it will enter your bloodstream. Then it will go to your thyroid and to any thyroid cancer sites. Most of what your thyroid tissue does not absorb will leave your body in your urine and stool. The I123 iodine gives off gamma rays (like X-rays), which can be seen with special Nuclear Medicine cameras. This allows us to see residual thyroid tissue (small amounts that are left after thyroid removal surgery) and any other places where there is still thyroid cancer. What will happen during the survey? Images taken by a special Nuclear Medicine camera show dark areas where I123 is absorbed by the body. You will swallow a small capsule that contains a diagnostic dose (5 millicuries) of I123. The next day, we will use the Nuclear Medicine cameras to take pictures of your head, neck, and body to look for thyroid tissue and sites of thyroid cancer. You will lie on your back on a table for about 1 to 2 hours while the cameras scan your body. Page 1 of 4 Thyroid Cancer Survey Using I123

6 What are the side effects of the survey? Side effects from I123 are very rare. But there is a slight chance you might have some nausea the day you swallow the capsule. How to Prepare for Your Survey Diet You must be on a low-iodine diet for 1 week before your diagnostic thyroid cancer survey. For low-iodine diet instructions, please visit this website: If you do not use the Internet, please ask for a printed copy of these instructions. Medicines If you are a withdrawal patient, you will need to stop taking: Synthroid (Levothyroxine). Stop taking this for at least 6 weeks before your survey. Cytomel (Liothyronine). Stop taking this for 3 weeks before your survey. If you are a Thyrogen rtsh patient, you do not need to stop taking any of your thyroid medicines. You will receive Thryogen rtsh injections on 2 different days before you take the I123 capsule. On the Day You Take Your Dose of I123 Do not eat any solid foods or drink anything but water for 2 hours before and after your appointment. We encourage you to drink water. You will have a blood draw for lab tests just before you receive your diagnostic dose. These tests will check your blood counts and thyroid hormone levels. For Women You must tell us if you are pregnant. If you will receive I131 therapy after your thyroid cancer survey, you must have a blood pregnancy test before you take the radioactive iodine. This test will be done the day you take your I123 dose. It may be done again on the day of your I131 therapy. Please tell us if you are breastfeeding. Women who are breastfeeding may not receive radioactive iodine, since it may be passed to your child in your breast milk. Women who will have I131 therapy after their thyroid cancer survey usually must stop breastfeeding for 3 months before they take the I131 dose. Page 2 of 4 Thyroid Cancer Survey Using I123

7 After Taking Your Dose of I123 Do not eat any solid foods or drink anything but water for 2 hours after you swallow the iodine capsule. An empty stomach will ensure that your body absorbs enough of the radioactive iodine. Starting 2 hours after your dose, we encourage you to continue to drink plenty of water. Juice is OK too. Remember, you will need to return to Nuclear Medicine for a scan the day after you take your diagnostic dose of I123. Stay on your low-iodine diet while you are having your diagnostic scans and any iodine therapy. The Night Before Your Scan Use an over-the-counter laxative (one you can buy without a prescription) the night before your scan. Be sure to use one that does not contain iodine. Some iodine-free laxatives are Miralax, Bisacodyl, Milk of Magnesia, Magnesium Citrate, and Dulcolax suppositories. Taking a laxative to empty your bowel removes any extra I123 that has collected in your stool. Stool that contains I123 could give a false positive (incorrect) reading during your scan. After Your Scan If you are NOT having I131 therapy, you can resume your normal diet after your final scan. If you ARE having I131 therapy, you must wait to resume your normal diet for 3 days after you swallow the I131 capsule. Radiation Safety Precautions There are no special precautions required after you swallow the I123 dose. Who reads the thyroid cancer survey scan and when will I get the results? A Nuclear Medicine doctor will read your thyroid cancer survey scan. This doctor also will review any other scan results that you bring with you. If you are scheduled for I131 therapy after your thyroid cancer survey, the Nuclear Medicine doctor may also talk with you about some of the findings of your survey. If you are not scheduled for I131 therapy right away, the doctor who referred you for this test will give you your thyroid cancer survey results within about 3 days.
