La gentamicina es una medicina, un antibiótico para tratar o prevenir infecciones de las vías urinarias. Se puede administrar directamente en la vejiga con una sonda. A esto se le llama irrigación de la vejiga y se hace para dar tratamiento a las infecciones urinarias difíciles de tratar con antibióticos por vía oral. Cómo irrigar la vejiga de mi hijo con gentamicina? Puede usar la solución para irrigar ya preparada o la preparar en casa. Se la pondrá a su hijo a través de una sonda. La sonda se coloca en la uretra de su niño y llega hasta la vejiga, es el conducto por donde sale la orina. Se usa una sonda para drenar la orina de la vejiga de su hijo y luego para administrar el medicamento en la vejiga. Dónde obtengo la medicina? No todas las farmacias aceptan la receta de gentamicina para irrigaciones de la vejiga porque es una preparación especial. Qué suministros necesito para preparar la solución de gentamicina en casa? Gentamicina, (80 mg), frasco de 2 ml Cloruro de sodio 0.9%, frasco de 50 ml Aguja y jeringa, de 1 ml 30G ½" (calibre) 1 de 5

2 Irrigación de la vejiga con gentamicina Jeringa de pistón y émbolo Jeringa de 30 ml Aguja calibre 19 Toallitas con alcohol Envase para objetos punzantes Guantes no estériles Cómo preparar la solución para irrigación? Debe preparar la solución cada vez que la va a usar. Para prepararla: 1. Retire las tapas de los frascos de gentamicina y de cloruro de sodio. 2. Retire las toallitas con alcohol del sobre, limpie la parte superior de los 2 frascos con la gaza y deséchela. 3. Saque la jeringa y la aguja eclipse del paquete. Conecte la aguja a la jeringa girándola. 4. Quite la tapa de la jeringa. 5. Llene la jeringa con aire hasta que marque 0.6 ml. Se llena de aire jalando lentamente del émbolo hasta que llegue a 0.6 ml. 6. Introduzca la aguja en el frasco de gentamicina. 7. Inyecte todo el aire de la jeringa en el frasco. 2 de 5

3 Irrigación de la vejiga con gentamicina 8. Llene la jeringa con 0.6 ml de Gentamicina. 9. Retire la aguja del frasco de gentamicina. 10. Introduzca la misma aguja en el frasco de cloruro de sodio (sodium chloride). 11. Inyecte los 0.6 ml de gentamicina en el frasco y retire la jeringa. 12. Quite la aguja y tírela en el envase para objetos punzantes. 13. Saque la aguja calibre 19 y la jeringa de 30 ml del empaque. Conecte la aguja a la jeringa. 14. Llene la jeringa con 30 ml. de aire. 15. Introduzca la aguja en el frasco de 50 ml de gentamicina con cloruro de sodio. 3 de 5

4 Irrigación de la vejiga con gentamicina 16. Llene la jeringa con 30 ml de medicina jalando lentamente del émbolo hasta llegar a la raya de 30 ml en la jeringa. Para llenarla, inyecte de 5 a 10 ml de aire en el frasco alternando con extracciones de medicamento de 5 a 10 ml, hasta llenar la jeringa con 30 ml de medicina. 17. Saque la jeringa piston del empaque; retire el émbolo y el adaptador azul. Ponga el émbolo en la jeringa. 18. Inyecte los 30 ml de medicina en la jeringa de pistón por la punta. 4 de 5

Llene la misma jeringa de 30 ml y aguja calibre 19 con los 20 ml restantes de gentamicina con cloruro de sodio y agréguelos a la jeringa piston para obtener una dosis total de 50 ml. 20. Siga estos pasos: 1. Ponga la sonda en la uretra de su niño. 2. Drene toda la orina de la vejiga por la sonda. Deje la sonda en la vejiga. 3. Conecte la punta de la jeringa llena de gentamicina al extremo de la sonda. Empuje el émbolo de la jeringa para que la medicina entre a la vejiga. 4. 7 Patient and Family Education Gentamicin Bladder Irrigation What is Gentamicin bladder irrigation? Gentamicin is an antibiotic medicine that is used to treat or prevent urinary tract infections (UTIs). It can be given directly into the bladder through a catheter. This is called bladder irrigation. It is used to treat UTIs that are difficult to treat with antibiotics taken by mouth. How will I give my child Gentamicin bladder irrigation? You may get the irrigation already made or you may have to make it at home. It will be given to your child through a catheter. You will put the catheter into your child s urethra. The urethra leads from your child s bladder to the outside. It is what the pee comes out of. The catheter will drain the urine out of your child s bladder and then it will be used to put the medicine into your child s bladder. Where can I have my child s prescription filled? Not all pharmacies will fill prescriptions for Gentamicin for bladder irrigations because it must be specially made. If you cannot find a pharmacy to make it for you, the pharmacies at Seattle Children s Hospital and Seattle Children s Bellevue Pharmacy are able to provide the supplies so you can make at home. What supplies do I need if I make the Gentamicin irrigation at home? Gentamicin, (80 mg)/2 ml vial Sodium chloride 0.9%, 50 ml vial Needle and syringe, 1ml 30G ½" Piston syringe and plunger Syringe, 30 ml 1 of 5

8 Gentamicin Bladder Irrigation Needle, 19 gauge Alcohol prep pads Sharps container Non-sterile gloves How do I make the irrigation solution? You will need to mix the irrigation solution right before you use it each time. To make the solution: 1. Remove the caps from Gentamicin and sodium chloride vials. 2. Remove the alcohol pad from the package, wipe the tops of both vials with the pad and throw the pad away. 3. Remove the eclipse needle and syringe from package. Attach the needle to the syringe by twisting it on. 4. Remove the syringe cap. 5. Draw up air into the syringe to the 0.6 ml mark. You draw up air by slowly pulling back on the syringe until it stops at the 0.6 ml mark. 6. Pierce the Gentamicin vial with the needle. 7. Push all the air in the syringe into the vial. 2 of 5

9 Gentamicin Bladder Irrigation 8. Draw up 0.6 ml of Gentamicin into the syringe. 9. Remove the needle from the Gentamicin vial. 10. Using the same needle and syringe, pierce the sodium chloride vial. 11. Push all of the 0.6 ml of Gentamicin into the vial. 12. Take out the needle, and put it in a sharp s container 13. Remove the 19-gauge needle and the 30 ml syringe from packaging. Attach the needle to syringe. 14. Draw up air into the syringe to the 30 ml mark. 15. Pierce the 50 ml vial of Gentamicin in sodium chloride with the needle. 3 of 5

10 Gentamicin Bladder Irrigation 16. Draw up 30 ml of medication by slowly pulling back on the syringe until you see the medication at the 30 ml line on the syringe. You will need to push 5 to 10 ml of air into the vial alternating with removing 5-10 ml of medication until 30 ml of medication is in the syringe. 17. Remove the piston syringe from packaging; remove the plunger and blue adapter. Replace the plunger into the syringe. 18. Place the 30 ml of medication into the piston syringe through the tip of the syringe 4 of 5

Using the same 30 ml syringe and 19-gauge needle, draw up the remaining 20 ml of Gentamicin in sodium chloride and place in the piston syringe for a total dose of 50 ml. 20. Place used needles in a sharp s container. Free Interpreter Services In the hospital, ask your child s nurse. From outside the hospital, call the toll-free Family Interpreting Line Tell the interpreter the name or extension you need. How do I give my child gentamicin bladder irrigation? Follow these steps: 1. Put the catheter into your child s urethra. 2. Catheterize the bladder and let all the urine drain out. Leave the catheter in the bladder. 3. Attach the gentamicin filled catheter tip syringe into the end of the catheter. Push the syringe plunger in and push the medicine into the bladder. 4. Leave the syringe attached to the catheter and gently pull the catheter and syringe out of the bladder. The medicine will stay in the bladder until the next catheterization.
