Please take a few moments to share your thoughts and opinions about your child s elementary school and our elementary standards-based report card. Please follow this link to access the survey on Survey Monkey: We would like to hear from every elementary parent in our school district. Once you have completed the survey, please initial and return this sheet to your child s teacher indicating that you have had an opportunity to complete the online survey. If you prefer, you may complete the attached paper survey instead and return it to your child s elementary school. We appreciate you for taking the time to offer us some feedback about our schools.
