1 Health Net Access News Summer 2014 Who to Call When You Need Care Hopper Helping kids learn how to stay healthy and fit. When you re not feeling well, you just want to be at home. But you may need to turn to your medical home. What s your medical home? It s your primary care physician s (PCP) office. Your PCP is your main doctor. When you build a relationship with your PCP, you ll have a place to call or go with all your health problems and questions. Your PCP s office won t always be open, though. That s when Health Net Access Nurse24 SM service can help. Call to speak with a nurse who can help you decide if: Your problem can wait until your PCP s office opens. You need to go to urgent care. You need to go to the emergency room (ER). Of course, some symptoms deserve a direct call to 911 or a trip to the ER. Get immediate help if you have: Chest pain. Trouble breathing. Severe bleeding or head trauma. Loss of consciousness (fainting). Sudden loss of vision or blurred vision. 2 After Your Hospital Stay Hospitals help you get well. But you don t want to go back to the hospital after you just left. Follow these tips to avoid going back to the hospital. Know what medicines you need to take and how to take them. Ask the nurse or doctor what symptoms you should watch out for when you get home. Make an appointment to see your doctor within 7 days of leaving the hospital. When you see the doctor after leaving the hospital, put all of your medicines in a bag and take the bag with you to the doctor s office. Ask the doctor any questions you have. Write your questions down to help you remember. Make sure you understand all of your doctor s instructions before you leave the office. If you have any questions after you leave the hospital or you need help getting ready for your doctor s appointment, call Nurse24 SM at Ask the nurse in the hospital what you should do if you feel worse when you get home. Before you leave the hospital, make sure you understand all instructions. If you don t understand them, ask the nurse to explain it to you until you do. Ask your doctor to explain your condition that brought you to the hospital. You will need to tell the doctor you see after you leave the hospital about why you were in the hospital. 3 Manage Diabetes for Better Health Managing your diabetes can help you stay healthy. And it can lower your risk for heart attack, stroke and other problems caused by diabetes. To help control your diabetes, here are some steps you can take: Every day. Eat well. Your doctor can help you create a meal plan to follow. Exercise on most days of the week. Try 10-minute walks 3 times a day. Take your medicine as your doctor told you even when you feel good. At least twice a year. See your doctor and have: a blood pressure check, a foot check and a weight check. Twice a year. Have an A1C test. This blood test measures your average blood sugar level over time. Ask your doctor what your goal number should be. Once a year. Have a: Blood test to check your cholesterol and other things. Complete foot exam. Dental exam. Dilated eye exam. Urine test. Flu shot. 4 Be the Boss of Your Asthma Asthma doesn t go away. If you have it, you will always have it. But you can take charge of it. You need these 3 things: 1. An action plan. You can help your doctor write out a plan made just for you (or for your child). This plan lists your asthma medicines. It tells you when to use them. It also tells you what to do if your asthma feels worse. 2. A medicine for daily control. Asthma gets worse when your airways become swollen. Using a control medicine every day can help prevent that. Most people breathe in this medicine through an inhaler. 3. A rescue inhaler. This is different from your daily control medicine. You only use a rescue inhaler when your asthma feels worse. Rescue medicine opens up your airways fast. If you need to use your rescue inhaler more than 2 days a week, call your doctor. You might need to change your action plan. 7 Controle la Diabetes para una Mejor Salud Controlar la diabetes puede ayudarle a mantenerse saludable. Para ayudarle a controlar la diabetes, estos son algunos pasos que puede dar: Todos los días. Coma bien. Su médico puede ayudarle a crear un plan de comidas para que lo siga. Haga ejercicio la mayoría de los días de la semana. Trate de caminar 10 minutos 3 veces al día. Tome sus medicamentos como le dijo el médico, aunque se sienta bien. Al menos dos veces al año.
