1 Grades 9 th through 12 th 1260 West Esperanza Street San Bernardino, CA Phone: Incoming 9 th or 10 th Grade Application Packet Purpose: The application will provide information in helping to determine student eligibility to Middle College HS for Directions: For students currently attending a SBCUSD school and living within the boundaries of the SBCUSD please provide: 1. Completed Application form 2. Handwritten Essay (form attached) NOTE: A good essay supports a strong application 3. Proof of Address (current utility bill, rental agreement, etc.) For students who live in the SBCUSD boundaries but did NOT attend a SBCUSD school in : 1. Completed Application form 2. Handwritten Essay (form attached) 3. Verifiable Proof of Address (current utility bill, rental agreement, etc.) 4. Current Official Transcript 5. Attendance Record for Standardized Test Scores (i.e. CST's, Stanford-9, ELA/mathematics assessments, other) for English Language Arts and Math for grades 5, 6 and Discipline Records for 8 th grade Application Review: 1. Incomplete applications may not be considered. 2. Applications must be turned in person to the MCHS office located at 1260 W. Esperanza St., SB, CA Student essays will be assessed by MCHS. 4. Proof of address is subject to verification by district. P.O. Box information will not serve as proof of address. 5. All student records will be evaluated according to MCHS enrollment criteria. 6. Siblings/relatives of current or past MCHS students are not guaranteed acceptance to MCHS. 7. Qualified applicants will be invited to an interview in spring. This continues the application process. 8. Acceptance or denial letters will be sent out in mid to late May. No applicant is guaranteed acceptance. 9. Students accepted to MCHS will be required to attend a one day student orientation in early June. 10. Thank for taking the time to apply to Middle College High School. Reporte de Asistencia Escolar para el año escolar Puntaje de los Exámenes Estandarizados (i.e. CST Scores, Stanford-9, evaluaciones de matemáticas e inglés, etc.) en las materias de Ingles y Matemáticas para los grados 5, 6, y Expediente de disciplina del 8º grado Reviso de su solicitud: 1. Solicitudes incompletas no se considerarán para revisión. 2. Su solicitud se debe de entregar en persona a la oficina de MCHS ubicada al 1260 W. Esperanza St., SB, CA El ensayo (essay) del estudiante será evaluado por un equipo de revisión que consiste de miembros de MCHS. 4. Su comprobante de domicilio es sujeto a verificación por el distrito. Un apartado postal no servirá como comprobante de domicilio. 5. El expediente académico del estudiante se evaluará según los criterios de inscripción de MCHS. 6. No se garantiza aceptación a MCHS a hermanos o parientes de estudiantes que actualmente están asistiendo o se graduaron de MCHS. 7. Los estudiantes que califiquen serán invitados a una entrevista en la primavera. La entrevista es parte del proceso de solicitud. 8. Se le dejar saber por correo sí, sí o no, fue elegido su estudiante a MCHS. Las cartas se enviaran a mitad del mes o los últimos de mayo. A ningún solicitante se le garantiza que será aceptado. 9.
