3 Hispanic/Latino Health Coalition of Northwest Ohio Members The Hispanic/Latino Health Coalition of Northwest Ohio was formed in 2005 to identify and address the health needs of Hispanics/Latinos who are here year round as well as the seasonal farm workers in Defiance, Fulton, Henry, and Williams counties. This group meets the fourth Tuesday of every month. You need to be on time for your appointment. Many offices have rules such as: if a patient does not come for their first appointment, they cannot use that office again. 2. If you made an appointment and you are unable to go to it, you need to call to cancel it and make another appointment. 3. You need to come on time even if you have to wait a long time after you arrive. 4. When you enter a medical office, walk up to the receptionist s window and stay there until someone speaks to you. 5. Give your interpreter request card to the nurse or ask for an interpreter. 6. Tell them your name, your doctor s name, and what time your appointment is. 7. It is not necessary to give your social security number before you receive care. Do not worry if you do not have one. 8. People here often do not speak to each other when sitting close such as in a waiting room. This may be uncomfortable for you, but stay there anyway. 9. A nurse will come to the waiting room and call your name when it is your turn to see the doctor. 10. If you go to the hospital, ask for an application for financial assistance. You may qualify for help to pay for these medical services.
