1 CENTINELA VALLEY UNION HIGH SCHOOL DISTRICT SHORT TERM INDEPENDENT STUDY CONTRACT PROCEDURES Per Board Policy and Administrative Regulation 6158: The superintendent or designee shall ensure that a written independent study contract, as prescribed by law, exists for each participating student. Individuals with disabilities, as defined in Education Code 56026, may participate in independent study, provided that his/her Individualized Education Program specifically stipulates such participation. (Education Code 51745) The minimum period of time for any independent study option shall be five consecutive school days The maximum length of time which may elapse between the time an assignment is made and the date by which the student must complete the assigned work shall be 3 weeks, unless extended by the superintendent or designee. Students in independent study shall have access to the same services and resources as any other student attending school To assure that the independent study contract for each student contains all the legal requirements, the CVUSD Short Term Independent Study Contract has been developed. The form must be completely filled out for each student on independent study at the time the student is placed on independent study. Please note the following: BEFORE STUDENT LEAVES: COMPLETE THE CONTRACT Contract Coordinator: Student s counselor Reason for Independent Study Contract: Often this is family travel (must be 5 days or more) Independent Study Duration: Specify the start and end dates Number of Independent Study Days: Indicate the number of days the student will be absent. Do not include weekends or holidays. A student should not be marked excused on attendance register if he/she is on independent study. Maximum amount of time allowed for completion of assignments: The same number of Short Term Independent Study days. The Work Due Date: Usually the date the student will return to school. Manner in which work is returned: e.g. meeting, , telephone, or other forms of communication with the teacher Frequency of work review: Usually this is upon student s return to school however if the student will be meeting with the teacher during the time he/she is absent from school indicate the appointment date and time. Work must be submitted to: Indicate name of person and office location. Assignments which can be missed before evaluation: Usually for Short Term Independent Study this is a zero. Signature and Date Lines: Signed and dated by all required parties to the agreement before the student begins Independent Study. (MUST have an administrator s signature on the day the contract is signed) The Independent Study Contract Coordinator is usually the counselor. The coordinator is responsible to distribute the contract to the student s teachers and provide the student the assignments to receive credit. ASSIGNMENT AND EVALUATION FORM (COMPLETED BY THE CLASSROOM TEACHER) List of Resources and Study Materials Available: Materials and personnel made available to the student Objective: Focused Learning Target (FLT)/ Students will be able to (SWBAT) Method of Study: Student assignments and/or activities Method of Evaluation: How will you evaluate this student? Report, presentation, activity log, test, etc. Course Credits: Maximum points available for the assignment or activity AFTER STUDENT RETURNS: Classroom Teacher Signature (BOTTOM OF ASSIGNMENT AND EVALUATION FORM) Date Assignments/Activities Evaluated: Indicate the date assignments and activities were evaluated Course Credits: Indicate the total number of points awarded for work submitted Attendance Credit: The classroom teacher must determine the total number of days for attendance credit. This should be determined based on the completion of the assigned work submitted. Attendance Clerk: On the bottom of the contract, where it states FOR OFFICE USE ONLY, place the sum of the days for attendance awarded by each of the Independent Study Contract classroom teachers. Divide that sum by the number of classes a student has per day. The result is the number of days for attendance apportionment that the student will earn. Enter the attendance in PowerSchool. Keep the contract, signed Assignment and Evaluation Form, with homework samples for a three-year period.

2 CVUHSD SHORT TERM INDEPENDENT STUDY FLOWCHART Parent requests appointment with the counselor to discuss the option of Short Term Independent Study for his/her student. Short Term Independent Study may be assigned for absences of 5 to 20 school days. If the student has an IEP, an amendment must be developed to place the student on Short Term Independent Study. Parent & student meet with counselor to sign the Short Term Independent Study Contract and obtain an Assignment and Evaluation Form from each classroom teacher. Short Term Independent Study Contracts must be signed by all parties on the same date. A copy is given to the attendance clerks, parent and student. Student returns to school and submits assignments to the counselor, no later than on the due date indicated. Counselor secretary makes a copy of the work submitted and returns originals to the student. Student submits original assignments to the classroom teachers. Classroom teachers evaluate student work submitted and complete the bottom portion of the Assignment and Evaluation Form. Classroom teachers return the completed Assignment and Evaluation Form to the counselor secretary. Counselor secretary submits completed Assignment and Evaluation Form and copies of the student work to the attendance clerks. Attendance clerks calculate the total days of attendance credits earned, reflect it in PowerSchool and file the original Short Term Independent Study Contract, completed Assignment and Evaluation Form and copies of the student work for 3 years.

Date / / Time Upon Student s return to school / / Work must be submitted to (name of person) Location Number of assignments which can be missed before there must be an evaluation of whether it is in the student s best interest to continue in Independent Study: The Independent Study Contract is a voluntary alternative to classroom instruction, consistent with CVUSD s course of study, which allows students to earn attendance credit through supervised ongoing instructional activities which are agreed upon in advance by the school, parents and students. Independent Study may be assigned for absences of 5 or more school days. It is the student s responsibility to complete all assignments and projects according to this agreement. As the parent or guardian, it is your responsibility to monitor your student s progress. According to the Independent Study Contract, all written assignments must be complete and returned to your student s short-term independent study contract coordinator. All assignments must be turned in by the Work Due Date indicated above. The supervisor will forward the completed assignments to the appropriate teacher for final evaluation and the issuance of attendance credits. All nonwritten assignments must be validated by the student s parent or guardian. Independent Study requires full documentation for the state auditors. Therefore, samples of work completed during the period of this contract must be on file in the school attendance office for a period of three years. The major objective of this contract is to enable the student to keep current with his/her studies for the period covered by this contract. The Assignment and Evaluation Form identifies the objectives to be covered and the study and evaluation methods. Students in Independent Study have access to the same services and resources as is available to other students in their school. While your student is fulfilling this Independent Study contract, all of the student s education will occur off campus. The parent of guardian assumes all liability for the safety and welfare of the student and for all of the student s educational activity. This contract is for the period indicated and it may not be extended. If it becomes necessary for your student to continue in Independent Study, this contract must be completed and a new contract must be initiated and approved. Estudio Independiente puede ser asignado por ausencias de 5 o más días de clases. Es responsabilidad del estudiante completar todas las tareas y proyectos de acuerdo con este contrato. Como padre o tutor, es su responsabilidad supervisar el progreso de su estudiante. De acuerdo con el Contrato de Estudio Independiente, todas las tareas escritas por el estudiante deben ser completadas y devueltas al coordinador de estudio independiente por corto plazo. Todas las tares deben ser entregadas en la fecha que la tarea es debida como indicada anteriormente. Todas las tareas no escritas deben ser validadas por el padre o tutor del estudiante. El Estudio Independiente requiere documentación completa para los auditores estatales. Por lo tanto, muestras del trabajo realizado durante el período de este contrato deben estar en el archivo de la oficina de asistencia de la escuela por un período de tres años. El principal objetivo de este contrato es permitir que el estudiante se mantenga al día con sus estudios por el período cubierto por este contrato.
