1 FIVE EASY ADMISSION STEPS 1. Complete the Family/Student Form along with a one-time $100 application fee.. Fill out the Authorization to Release Records. Saint Ann School must have copies of the following items: a. Birth Certificate b. Baptismal Certificate (if applicable) c. Medical Insurance Card d. Original Copy of the student s State of Florida School Entry Health Exam (DH 3040) e. Original Copy of the student s Florida Certification of Immunization (DH 680) 3. Schedule a time for student to take a placement exam. 4. Complete and sign the Financial Commitment Form. a. Once the Admissions Committee has reviewed your application and your child has been accepted, pay the $450 enrollment fee. b. Setup a tuition payment plan using the FACTS Tuition Management System at: or by clicking on the FACTS link on the right hand side of the school website at Saint Ann School does not accept tuition payments by check or credit card unless you are paying the tuition in full. *A nominal fee, administered by FACTS, applies when you enroll in a tuition payment plan. 5. Financial Aid Requests are made by completing the FACTS Grant and Aid Assessment Application online at or by clicking on the FACTS link on the right hand side of the school website at A FACTS Grant and Aid Assessment is required for consideration for scholarships, tuition assistance, parish subsidy, etc.

This fee will be waived if a student participates in the school raffles according to the guidelines presented by the school. One way we do this is through our Grant & Aid Assessment service, which allows schools to award financial aid with confidence to the families who truly need it. We work with schools to create a custom application for families, so FACTS can collect the financial data that matters. 6 DID YOU KNOW ABOUT THESE TWO K-1 SCHOLARSHIPS IN FLORIDA? Step Up For Students is a nonprofit organization that helps administer the Florida Tax Credit Scholarship (FTC) and the Personal Learning Scholarship Account (PLSA). The scholarships are awarded on a first-come, first-served basis and empower families to customize their children s education. 1 FLORIDA TAX CREDIT SCHOLARSHIP PROGRAM (INCOME-BASED SCHOLARSHIP) The FTC allows low-income families to choose between a scholarship worth up to $5,7 to help cover private school tuition and fees, or a scholarship worth up to $500 to assist with transportation costs to attend a public school in another county. (These scholarship amounts are for the school year. The private school scholarship amount may increase for the school year.) PERSONAL LEARNING SCHOLARSHIP ACCOUNT (SPECIAL NEEDS SCHOLARSHIP) The PLSA allows parents to customize the education of their children with special needs by directing money toward a combination of approved programs and providers including approved private schools, therapists, specialists, curriculum, technology even a college savings account. The scholarship amount varies, but the average amount for most students for is $10,000 (and may increase in ). Students with the most severe disabilities are eligible for a higher scholarship amount. Families receiving a PLSA through Step Up For Students will have access to a Personal Learning Plan Advisor. Students entering kindergarten must be 5 on or before Sept. 1. Students entering first grade must be 6 on or before Sept. 1. Students can participate in the PLSA as part of home education, but they are not eligible while enrolled in a public school or receiving any other state-sponsored scholarship. Students can continue to receive the scholarship until they graduate from high school, reach age or return to public school. Students must be diagnosed with one of the following: autism, cerebral palsy, Down syndrome, Prader-Willi syndrome, Spina bifida, Williams syndrome or an intellectual disability (severe cognitive impairment). Or students who are deemed high risk due to developmental delays and who are not older than 5 on Sept. 1 may be eligible for the year they are applying. Students need either a current Individual Education Plan (IEP) from a Florida school district or a formal diagnosis from a Florida licensed physician or psychologist. Children in foster care or out-of-home care, or who are homeless also may be eligible for either of these TWO scholarships.
