9 Manifestaciones clínicas Previo a la disponibilidad de pruebas de cernimiento como parte del cuidado de salud, se identificaban muchas mujeres con VIH por manifestaciones ginecológicas mas severas Vulvo-vaginitis por Candida Citología cervical anormal Inflamación pélvica Ulceras genitales (HSV, chancroid, syphilis, idiopaticas) Síntomas menstruales VIH y el contexto social Es esencial considerar la relación entre la pobreza, la falta de educación, el uso de substancias, enfermedad mental, abuso físico y emocional, y enfermedades de transmisión sexual particularmente afectando el riesgo y la progresión en las mujeres. 9

10 Evaluación inicial: mujer con VIH Examen físico completo Laboratorios de rutina Confirmar HIV Carga viral, Genotipo CD4, CD8 Evaluar para otras ETS (GC/Chlamydia, Syphilis, Hepatitis, HSV) Pap Smear, HPV Evaluación inicial: mujer con VIH Evaluar red de apoyo social Consejería de reducción de riesgo Planes de revelación del diagnóstico Opciones reproductivas Consejería pre-concepción Indicaciones para HAART 10

11 Otras evaluaciones Síntomas de otras infecciones (OI) Historial de terapia, y efectos secundarios Uso de sustancias (alcohol, cigarrillos, otras drogas) Prácticas sexuales, contracepción Evaluar para violencia doméstica Otros labs: LFT s, serologías para Toxo, CMV, lípidos (T Chol, HDL, LDL, TGC), posibles pruebas de inflamación (HS-CRP) Monitoreo Cada 6 meses si los CD4 s >500 Cada 3 meses si hay síntomas o en terapia HAART En 4-6 semanas si comenzó terapia recientemente. Evaluar para síntomas y laboratorios 11

12 Antes de un embarazo El cuidado pre-concepción es importante para las mujeres que viven con el VIH, sobre todo si están tomando medicamentos antivirales Las opciones de terapia (HAART) son diferentes si se considera un embarazo Para toda paciente en cuidado de salud, un embarazo es una posibilidad real Todo profesional de salud debe asegurarse de evaluar para posibles embarazos Birth of a star Consejería pre-concepción para mujeres viviendo con el VIH Discutir intenciones de embarazo durante el cuidado longitudinal Incluir información sobre metodos contraceptivos para reducir embarazos no planificados Incuir información sobre prácticas de sexo más seguro, el descontinuar el uso de drogas, alcohol y cigarrillos Evaluar condición HIV y necesidad de uso de HAART Escoger HAART considerando eficacia, potencial teratogénico y efectos secundarios PHS Perinatal Guidelines September 14,

Aprobada también para la prevención de cáncer anal Cáncer invasivo de cervix El cáncer invasivo de cérvix es una de las condiciones que definen SIDA. Responde menos a tratamiento. Hace metástasis a lugares poco usuales. Tiene más riesgo de recurrencia. 15

16 Cáncer anal en mujeres con VIH Nuevos casos en los EU (2008): 5,070 (3,050 en mujeres) La prevalencia de VPH en el tracto anal en mujeres VIH + es de un 60-80% (vs 15-20% en la población general) El ACOG recomienda cernimiento con citología anal cada 2-3 años 16% de citologías anormales en mujeres con VIH (vs 4% en mujeres negativas) 9% con displasia severa Las pacientes con citología anormal deben ser referidas para anoscopía de alta resolución (HRA) Métodos anticonceptivos para la mujer infectada con el VIH Métodos de barrera Anticonceptivos orales (hay interacciones con ARVs, se recomienda método adicional) Ortho-Evra (parcho semanal) Nuva Ring (anillo vaginal) Contraceptivos Inyectables (Depo-Provera): información contradictoria sobre interacción con ARVs Dispositivos intrauterinos: evaluar factores de riesgo para ETS Esterilización quirúrgica 16

17 Métodos contraceptivos y el VIH Los métodos hormonales pueden aumentar el riesgo de VIH en mujeres de alto riesgo Información conflictiva sobre contraceptivos hormonales y progresión de enfermeded: 2 estudios con 370 mujeres no encontraron impacto mientras que otro estudio con mujeres sin terapia encontró progresión con contraceptivos orales (13.2/100 mujeres años), comparado con el IUD Cu7 (8.6/100 mujeresaños); HR:1.5; 95% CI ) Los no-nucleósidos e inhibidores de proteasa alteran el citocromo P450, y pueden afectar los niveles de estradiol y progestágenos y afectar la eficacia de los anticonceptivos orales En general las mujeres con VIH pueden usar todos los métodos reversibles.
