3 Student and Parent Expectations Students must be dropped off in the appropriate parking lot between 7:45-7:55am. Program Parent Permission/Emergency Medical Release This document serves as a permission form and emergency medical release to allow your child to participate in the Citrus College P.A.G.E. Mathematics Program to be held on the Citrus College campus from 8am-11:45am during the dates of June 22- July 17, This summer program will take place in various rooms at Citrus College and will be conducted by instructors hired by Citrus College. Written permission is required in order for your child to participate in this program. Please fill in the information below to give your child permission to participate. Students will be asked basic questions about their interest and perceived ability in mathematics, and, at the end of the program about their experiences during PAGE. Participation in the survey is voluntary and requires your permission. There are no negative consequences related to refusal to participate in this survey. Furthermore, the information collected will be used for the purpose of evaluating the PAGE Program and all responses will remain confidential.
