The Campbell Union High School District teachers, administrators and support staff look forward to meeting you and your student as we work together on your child s educational success. We understand that you have high expectations of our District. As specialists in high school education, the Campbell Union High School District is focused on providing a safe, innovative, challenging, standard based program for all of our students, while simultaneously offering diverse opportunities in extracurricular activities and clubs. Our schools continue to focus on developing our students so that they are ready for college and prepared to succeed in college or the work force. This year, we will continue to explore opportunities to expand AP offerings in our schools, increase engineering pathways, and change graduation requirements to ensure more of our students will be eligible to attend college at either a UC or CSU campus. Additionally, we will be investing in new technology in all of our classrooms. We will be hosting sessions at all of our school sites this year to educate parents on the new Common Core State Standards. This summer, we are providing professional development to all of our faculty to embrace this change, and to provide them with time and resources to implement these new standards. Later this month, our Board will be contemplating the adoption of a resolution to renew the parcel tax that our community first approved in This tax provides $4.9 million of funding to our schools, all of which is spent on teachers and programs to support students and none of which can be taken by Sacramento. I want to continue to encourage you to be active in supporting your student s education. We know that setting high expectations, providing consistent encouragement, and participating and overseeing your student s education is vital to their success. We will continue to provide you with access to SchoolLoop, a program which will send you daily s with your child s homework assignments on one easy page to read. This high school graduation requirement was authorized by Senate Bill 2 of the Statutes of The stated purpose of the CAHSEE is to help improve student achievement in high school and to make sure that graduates meet identified state content standards in reading, writing and mathematics. At the time of this writing, the law prohibits 9 th graders from taking the test, requires all 10 th grade students to take the test, one time only, during the spring semester, and stipulates that 11 th and 12 th graders shall have multiple opportunities to take the test during the school year. Testing dates for the CAHSEE are designated by the State Superintendent of Public Instruction and are the same all over California. For the school year, the designated dates are; November 5 th & 6 th 2013, February 4 th & 5 th 2014, March 18 th & 19 th 2014, and May 13 th & 14 th CAHSEE testing for sophomores is once only during the spring semester, for juniors it is twice a year, and for seniors it is available three times a year. For all dates, the English Language Arts portion of the test is given on a Tuesday and the Mathematics portion on a Wednesday. The staffs at our schools are dedicated to making sure students receive the instruction they need to help them pass such tests. Students will be offered opportunities for assistance as they are needed. It is important that we work together to help all students succeed. 5 Dear County and District Superintendents and Charter and State Special School Administrators: ATHLETIC TEAM INSURANCE The California Department of Education strongly supports student participation in athletic activities. Some students may be unable to participate in these activities because they are unable to afford health insurance. This letter is seeking your assistance in informing families about the low-cost resources available to them. California Education Code Section (b) requires: The statement described in subdivision (a) shall also be incorporated into any other letters or printed materials, in boldface type of prominent size, that contain the name or logo, or both, of the school district and are sent to members of school athletic teams to inform them of the provisions of this article, or any other applicable provision of state law, regarding the provision of insurance protection. Under state law, school districts are required to ensure that all members of school athletic teams have accidental injury insurance that covers medical and hospital expenses. This insurance requirement can be met by the school district offering insurance or other health benefits that cover medical and hospital expenses. Some pupils may qualify to enroll in no-cost or low-cost local, state, or federally sponsored health insurance programs. Information about these programs may be obtained by calling the Healthy Families and Medical Programs Information Line by phone at Existing law requires local educational agencies (LEAs) to provide insurance protection to members of athletic teams for expenses resulting from accidental bodily injuries. The law also requires LEAs to pay for the costs of that insurance. Assembly Bill 2684 (Chapter 108, Statutes of 2006) requires LEAs operating interscholastic teams to inform parents that students participating in sports activities must have proof of health insurance. This bill informs parents of the availability of low or no cost government supported health insurance programs such as Medi-Cal, Healthy Families, or in some counties the Healthy Kids programs. Unfortunately, some material accessible via the Internet may contain items that are illegal, defamatory, or potentially offensive to some people. Access to the Internet, and other computer activities is given as a privilege to students who agree to act in a considerate and responsible manner. We require that students read, accept, and sign the following rules for acceptable on-line behavior. 1. Students are responsible for good behavior on the Internet just as they are in a school building. General school rules for behavior and communications apply. 2. Network administrators may review files and communications to maintain system integrity and ensure that users are using the system responsibly. Users should not expect that files will be private. 3. 10 Software Code of Ethics Students All employees and students of the Campbell Union High School District shall use software only in accordance with its license agreement. Unless otherwise noted in the license, any duplication of copyrighted software except for backup and archival purposes is a violation of federal law and Campbell Union High School District policy. This signed Code of Ethics will be filed with the student s records at each school. 1. I will use software according to the provisions of the license agreements. 2. I will not make unauthorized copies of software under any circumstances. 3. I recognize that the Campbell Union High School District will not tolerate, nor allow, the use of any illegal software copies on its computers. 4. I understand that anyone found copying software other than for backup or archival purposes is subject to disciplinary actions, up to and including dismissal or expulsion. 5. I understand that anyone found making illegal software copies may be subject to civil and criminal penalties up to $250,000 per work copied and/or expulsion from Campbell Union High School District. 6. I will report any suspected misuse of software to the District s Technology Services Department. 13 Campbell Union High School District Support Services Annual Pesticide Notification Assembly Bill 2260 (Healthy Schools Act of 2000) requires annual notification to all parents and staff of the names of all pesticides that are planned for use during the year at school sites. This includes the use of herbicides. The Campbell Union High School District has adopted an Integrated Pest Management approach. 15 Campbell Union High School District Major Reminders!!! From the District Nurses If your child will need to take medication during the school year, please take the AUTHORIZATION TO ADMINISTER MEDICATION form in this packet and have your physician fill it out and sign it. Return it to the health office at your child s school after you (the parent) have signed it. This includes all medications, even over-the-counter medications. The education code does not allow unlicensed personnel to give any medication unless we have written authorization from the parent/guardian and a signed physician s order. Students are not allowed to carry medications on their person unless it is for life saving medications. If your student needs to carry such medication, please fill out the authorization to carry medication form found in this packet. Physician signatures, as well as parental signatures are required. PLEASE, if your child has a major or chronic health condition, notify the health office. We have staff trained to assist in making the school environment as safe as possible. Such health conditions may include diabetes, seizure disorders, mobility difficulty, asthma, cardiac conditions or any other problems. If you have any questions, please contact the health office. Be sure to completely fill out the Blue Emergency Form. Accurate and up-to-date information allows us to contact you or your assigned representative in the event of an emergency. SHOTS UPDATE Varicella Any student who is enrolled for the first time into a California school MUST show evidence of 1) completed immunization for Varicella, or 2) evidence that they have had the disease (Chicken Pox). Documentation that an individual has had the disease must be obtained from a physician and so stated on the Yellow Immunization Card. Verbal report by parents/guardians does not constitute proof of the disease. TDaP Requirement All children entering 9 grade should have had the TDaP Vaccination as required by California Health and Safety code. No new documentation is required if previous school documentation has been completed. Tuberculosis Screening Tuberculosis skin testing is required for all students transferring from outside Santa Clara County and entering any school in Santa Clara County. This skin test must have been completed within six months prior to entry. A chest X-ray stating a person is free of communicable disease is acceptable. A tine test (Prong test) is not acceptable.

Estudios para detectar la tuberculosis Prueba de la tuberculosis es necesaria para todos los estudiantes que se trasladan desde fuera del Condado de Santa Clara y entran en cualquier escuela en el condado de Santa Clara. Esta prueba de la piel debe de haber sido completadas dentro de los seis meses anteriores a la entrada. Una radiografía de pecho indicando que una persona es "libre de enfermedades contagiosas" es aceptable. Una prueba de dientes (puntas de prueba) no es aceptable.

17 CAMPBELL UNION HIGH SCHOOL DISTRICT NOTICE TO PARENTS, GUARDIANS, AND STUDENTS Federal, state and local privacy laws specify the restrictions for the release of student information. These laws recognize the appropriateness of releasing certain very limited information. Such information is called "Directory Information" and is defined by the California State Education Code as: "student's name, address, listed telephone number, date and place of birth, major field of study, participation in officially recognized activities and sports, weight and height of athletic team members; dates of attendance; degrees and awards received; most recent previous public or private school attended by student." Directory information about a student as specified in this definition may legally be released by the school to PTSA and other school related organizations, representatives of the news media, employers, prospective employers, private schools or colleges and, unless parents object, the military; however, it may not be released to any other parties without parent or guardian consent. A box on the Registration Card may be checked to preclude information being provided to the military. If parents or guardians desire that no information, including Directory information be released, they need to submit this request in writing to the school principal or designee. Further information on this matter may be obtained from the school counselor RELEASE OF STUDENT INFORMATION: The District does not release information or records concerning a child to non-educational organizations or individuals without parent consent except by court order, receipt of a lawfully issued subpoena, or when otherwise allowed by law. The following categories of directory information may be made available to various persons, agencies or institutions unless the parent or guardian notifies the District in writing not to release such information. Name, address, telephone, date and place of birth, major field of study, class schedule, class roster, photographs, participation in officially recognized activities and sports, weight and height of members of athletic teams, dates of attendance, diplomas and awards received, and most recent previous educational institution attended. (E.C et seq., and 49073) INSPECTION OF STUDENT RECORDS: State law requires that the District notify parents of the following rights which pertain to student records. (E.C , and 49069) 1. A parent or guardian has the right to inspect and review student records relating directly to his or her child during school hours or obtain a copy of such records within five (5) business days of his/her request. 2. Any parent who wishes to review the types of student records and information contained therein may do so by contacting the principal at his/her child s school. The principal of each school is ultimately responsible for maintenance of student records. 3. A parent with legal custody has a right to challenge information contained in his/her child s records. Any determination to expunge a student s record is made after a review of said record(s) by site administrators and certificated staff. Following an inspection and review of student records, the parent may challenge the content of the student s record. The right to challenge becomes the sole right of the student when the student becomes eighteen (18) years of age. The parent may file a written request with the Superintendent of the District to remove any information recorded in the written records concerning the child which is alleged to be: Inaccurate An unsubstantiated personal conclusion or inference. A conclusion or inference outside of the observer s area of competence Not based on the personal observation of a named person with the time and place of the observation. Misleading. In violation of the privacy or other rights of the pupil. Within thirty (30) days, the Superintendent shall meet with the parent/guardian and the certificated employee who recorded the information, if any, and if the person is still employed with the District, any sustain or deny the allegations. If the allegations are sustained, the Superintendent shall order the correction, removal or destruction of the information. If the Superintendent denies the allegations, the parent may appeal the decision to the Governing Board within thirty (30) days. The Board shall determine whether or not to sustain or deny the allegations. If the Board sustains the allegations, it shall order the Superintendent to immediately correct, remove or destroy the information from the written records of the student. (E.C ) INSPECTION OF STUDENT RECORDS (cont.): If the final decision of the Board is unfavorable to the parents, or if the parent accepts an unfavorable decision by the District Superintendent, the parent shall have the right to submit a written statement of objections to the information. This statement shall become a

18 part of the student s school record until such time as the information objected to is removed. Both the Superintendent and the Board have the option of appointing a hearing panel in accordance with Education Code sections to assist in the decision making. The decision as to whether a hearing paned is to be used shall be made at the discretion of the Superintendent or the Board and not of the challenging party. NO CHILD LEFT BEHIND ACT OF 2001: Under the NCLB, parents have the following rights: Information Regarding Professional Qualifications of Teachers, Paraprofessionals, and Aides: Upon request, parents have a right to information regarding the professional qualifications of their student s classroom teachers, paraprofessionals, and aides. This includes whether the teacher meets the state qualifications and licensing criteria for the grades and subjects s/he teaches, whether the teacher is teaching under an emergency permit or other provisional status because of special circumstances, the teacher s college major, whether s/he has any advanced degrees and the subject(s) of those degrees, and whether any instructional aides or paraprofessionals provide services to the parent s child and, if so, their qualifications. Districts shall also notify parents if their child had been assigned to or has been taught for 4 or more consecutive weeks by a teacher who is not highly qualified. Limited English Proficient Students: The Act requires prior notice be given to parents of limited English proficient students regarding limited English proficiency programs, including the reasons for the identification of the student as limited English proficient, the need of placement in a language instruction educational program, the student s level of English proficiency, how such level was assessed, the status of the student s academic achievement, the methods of instruction used in the programs available, how the recommended program will meet the student s needs, program performance, parent options to remove a student from a program and/or to decline initial enrollment, and expected rate of transition into classrooms not tailored for limited English proficient students. NOTICE OF NON-DISCRIMINATION: It is the policy of the Campbell Union High School District to provide equal opportunity for all individuals in education and in employment. Making reprisals, threats of reprisal, or implied threats of reprisal following a negative response. For example, either implying or actually withholding support for an appointment, promotion, or change of assignment, or suggesting that a poor performance report will be given. 5. Engaging in implicit or explicit, coercive sexual behavior which is used to control, influence or affect the career, salary, and/or work environment of another employee. SEXUAL HARASSMENT (cont.):

19 This policy applies to all phases of the employment and academic relationships, including evaluating, recruiting, testing, hiring, upgrading, promotion/demotion, transfer, layoffs, termination, rates of pay, benefits and selection for training. It is the stated policy of Campbell Union High School District to prevent and prohibit misconduct on the job, including sexual harassment by co-workers, subordinate employees, vendors, or supervisors. Any employee and/or student found to have acted in violation of the foregoing policies shall be subject to appropriate disciplinary action, including warnings, reprimand, suspension, demotion, and/or termination. SPECIAL EDUCATION: Under federal and state laws, free appropriate special education, both public and private, is available to pupils with exceptional needs. Information of these programs may be obtained from the school counselor, district psychologists or the Director of Special Programs and Pupil Personnel Services. 504 PLANS: Under federal and state law, 504 accommodations are available to pupils with identified needs. Information is available from the school vice principal or guidance advisors at the school site. COMPLAINT PROCESS: Any employee or student who believes that he/she has been subjected to sexual harassment, discriminatory practices, bullying, intimidation, or inappropriate procedures with respect to special education or other educational programs should follow the grievance process set forth in Board Policy 1312 through which is available in the principal's office at each school site and at the District Office located at 3235 Union Avenue, San Jose, CA. To the extent possible, such sites will be blocked on district computers with Internet access. The Campbell Union High School District sponsors a number of publications for the benefit of its students, and among these are school newspapers, web versions of school newspapers, district and school websites, and communication systems for students, parents and teachers. The Board of Trustees realizes that there has been a proliferation of social networking websites of various sorts and that anyone, at any time, can gain access to such sites and post whatever messages or images they like. The Board of Trustees further acknowledges that school staff cannot control what students or other individuals place on such websites, unless any such postings cause a substantial school disruption. Therefore, it shall be the policy of the Board that parents receive annual notification that the district and its schools do not monitor, endorse or authorize the use of any social networking site for use by students or student organizations, and that student use of the internet that causes a disruption or is likely to cause a disruption, even if that use does not occur at school, or during school hours, or on a school day, will be subject to disciplinary action, including suspension or expulsion. Parents should be encouraged in these notifications to closely monitor and supervise their student s use of the internet for their own safety and wellbeing. Before using the district s technological resources, each student and his/her parent/guardian shall sign and return an Acceptable Use Agreement specifying user obligations and responsibilities. In that agreement, the student and his/her parent/guardian shall agree to not hold the district or any district staff responsible for the failure of any technology protection measures, violations of copyright restrictions, or users mistakes or negligence. They shall also agree to indemnify and hold harmless the district and district personnel for any damages or costs incurred. Revised 6/2013
