1 July 23, 2015 Dear families of The Noble Academy, The staff of The Noble Academy is excited about the beginning of the School Year! We have enclosed information that will help you plan. Please read this letter carefully; it contains important information. Items below must be turned into The Noble Academy: **Due by August 7** Lunch form All students must have a lunch form on file even if they decline to eat breakfast and lunch. Once you receive your lunch status (free, reduced, full), you can choose to participate or decline enrollment in the food program. Please read the enclosed lunch form procedures form carefully. If the lunch form is not returned by August 7 th, the student will be charged the full amount for meals the first week of School. Emergency /Contact Form ( received at time of registration) DIPLOMA ( If attended non-cps school we will also need birth certificate) Photo Release Form (received at time of registration) Student/Parent/School Compact (received at time of registration) 2 Proof of address - Each year you must provide proof of residence to the main office. Please bring a current ORIGINAL utility bill, lease, or mortgage stub that states the name of the legal guardian of the student and his/her address (we ll make a copy and return the original). Reminder: students must live with their legal guardian in the city of Chicago in order to attend The Noble Academy. Proof of current physical exam not older than August 1, If you don t have a current physical, please schedule an appointment with your doctor. Fees- Books for the school year will cost a total of $180. The first $90 for semester 1 is due by July 31st. The second $90 for semester 2 will be due in January at the start of second semester. Orientation: Orientation for Freshmen parents and students is Monday, August 17 from 6:00 p.m. to 7:00 p.m. at The Noble Academy, 1443 N. Ogden. At this meeting, you and your child will receive information about the upcoming school year. It is important that both parents and students are present for orientation, as we will have specific sessions for both groups. Students need to be in complete uniform at Orientation. Students will be turned away if they are not in complete uniform.

2 Any student that has not taken the placement test by the 17th of August will be taking the tests after the orientation portion of the evening has finished. Testing will be from 7:00 p.m. to 9:00 p.m. At The Noble Academy, we start all events on time. Please arrive and be seated in the school auditorium no later than 6:00 pm. Please allow yourself plenty of time for traffic, weather, and parking. Parking is not available at the school. Street parking is extremely limited. Public Transportation is encouraged. Uniforms: Please read carefully: The Noble Academy s uniform will be strictly enforced. Students are required to wear the Noble Academy polo, light-khaki pants or skirt, black leather belt, and black professional shoes. Shirts Students are required to wear The Noble Academy polo shirts. Students may not wear anything underneath their school shirt except one white short-sleeve t-shirt and nothing over them except Noble Academy s cardigan. a. Shirts must be tucked whenever students are on school grounds. b. No more than one button on The Noble Academy s polo can be unbuttoned at all times. You may purchase uniform shirts for $20.00 and cardigans for $37.00.** Pants/skirts - Students will not be permitted to wear pants/skirts that do not comply with the dress code. a. Boys must wear light-khaki dress pants. Girls must wear light-khaki dress pants or skirts. Capri/ankle pants, spandex material, and corduroys are not permitted. b. Pants must not be too baggy or too tight nor torn or frayed. Pants must be worn at waist level. Other clothing must not be visible under pants. c. Pants may not have any pockets below the hips and may not have any external pockets. Cargo pants, Carpenter pants, Capri pants, Gaucho pants, jean khakis, and shorts are not permitted. Jean khakis = Stitching across the back pocket and studs on the side pocket. d. Skirts must be at least knee length; mini skirts and skorts are not permitted. Belts - Solid black leather belts must be worn with all pants and skirts. Pants and skirts must have belt loops. Belts must be worn at all times. a. Belts may not hang down. b. Plain small buckle without patterns, designs, or logos are allowed. c. No extra holes throughout the belt. Shoes- All - Black professional dress shoes ONLY. Students may not wear gym shoes of any type during the regular school day, except during gym class. All parts of the shoe, including laces, must be black. a. Girls dress shoes must cover more than half of the foot and must not have more than a 3 heel. Plain ballerina flats are preferred. b. Flip-flops, sandals, stacks, platform shoes, and gym shoes are not permitted. Students must keep their shoelaces in their shoes, and keep their shoes laced at all times.

3 Hair - Hair must be in a natural human hair color. a. No Designs of any kind are permitted to be in the hair or on the face. This includes a prohibition of any distracting or unprofessional hair styles. b. Mohawks, mullets, V cut, or designed cuts into boys hair are not permitted. c. Hair must be out of student s face, with a professional appearance. d. Female students are not allowed to shave the sides of their hair. Jewelry & Head Coverings a. Boys may not wear earrings and girls may wear earrings only in ears. b. Girls may only have one earring per ear. Earrings should be no larger than a quarter in any direction. c. Visible body piercings, tongue piercings, or band-aids covering piercings are not permitted. d. All necklaces must be tucked inside the shirt. e. Socks must be plain black, white, gray, or navy. Patterns and designs are not allowed on socks. f. Head accessories should be no larger than 2 in any direction. g. One bracelet or watch per wrist. Rubber bands with text are not permitted. Gym Uniforms - Gym uniform shirts can be purchased for $8, and gym shorts are $15. Reminder: Students lose points in gym for not wearing The Noble Academy gym shirt and shorts. Locks - Each child should purchase a lock and bring the lock with them on the first day of school. After he/she receives his/her schedule, he/she should bring the lock every day of gym class. See example lock below: **As always, if you are uncertain about what to buy, please call us before you purchase anything. If you are in doubt, save your receipt so that you may return any items that do not conform to the standards. We are happy to help you figure out what to purchase so that your son/daughter is in uniform. Since we have clearly articulated our dress code policy, the staff of The Noble Academy expects parental support in strictly enforcing the dress code.** Ventra Cards: Ventra student riding permits will be available on the first day of class. Students who need a Ventra permit should bring $2. Ventra cards should be registered on the Ventra website in case they are lost or stolen. School Supplies: We want students to value and take care of their books. The books your child will be given are to be returned at the end of the year. The only exceptions are workbooks, or books in which the students write; these cannot be sold back. We reserve the right to refuse any books that are poor condition and a fee will be charged to the student. Students who have not paid the 1 st Semester book fee will not receive their textbooks until the fee is paid.

4 Other Notes: Although it is not required, we highly recommend that students use a heavy-duty backpack and a pen/pencil case in order to keep their supplies organized. There may be other supplies that teachers will require from students and that list will be given at Orientation. We will do our best to make sure that any additional requests are minimal. Summer Reading: Upon completion of his/her placement test, your child was given the required book for summer reading and other assignments. Please make sure he/she completes the assignments, which will be collected the first day of school. Schedule for the First Week: Monday, August 24 through Thursday August 27: 7:25am- 3:10pm (building opens at 6:45am) Student who do not complete the summer assignments will be required to stay until 4:50pm. Friday, August 28: No Classes All students should meet in the cafeteria of our campus (1443 N Ogden) no later than 7:20am Monday, August 24. Beginning the first day, students are expected to be on time and in full uniform; the dress code will be enforced starting the first day. Students out of dress code will not be permitted into the building. There will be regular gym classes starting the first week. Students should bring their gym uniform which includes The Noble Academy Campus gym shirt and shorts on the first day of school. Por favor, anticipe tiempo para el tráfico, clima, y estacionamiento. No hay estacionamiento disponible en la escuela. Aparcamiento en la calle es muy limitado. Recomendamos transporte público. Uniformes: Por favor, lea atentamente: el uniforme de Noble Academy será estrictamente esforzado. Los estudiantes están obligados a llevar la polo de Noble Academy, pantalones de vestir color khaki o falda, cinturón de cuero negro y zapatos negros profesionales. Camisetas Estudiantes están obligados a usar la polo de The Noble Academy. Estudiantes no pueden usar nada debajo de la polo al menos que sea camisa de manga corta. Mujeres pueden usar pantalones de vestir o faldas color caqui. b) Pantalones apretados, grandes, o debajo de la cintura no serán permitidos. c) Pantalones no pueden tener bolsillos externos debajo de la cintura. Pantalones Cargo, Carpintero, Capri, Gaucho, mezclilla caqui y pantalones cortos no son permitidos. Mezclilla caqui= puntadas en los bolsillos y tachuelas en los bolsillos de al lado. d) Faldas deben ser a medida debajo de la rodilla. Faldas cortas y falda/short no son permitidas. Cinturón- Cinturones negro de cuero sólido se debe usar con su pantalón de uniforme y falda. Pantalones y faldas deben tener hebillas de cinturón. Cinturones son mandatorios a en todo momento. a) Cinturones no deben colgar. b) Hebilla pequeña sin diseños o logos están permitidos. c) Agujeros adicionales a lo largo del cinturón no son permitidos. Zapatos Todo zapado debe ser SOLAMENTE negro y profesional. Estudiantes no pueden usar zapatos de gimnasio durante el día, excepto durante la clase de gimnasio. El zapato completo debe ser negro incluyendo las agujetas. a) Zapato para niñas debe cubrir más de mitad del pie entero y el tacón no debe ser más de tres pulgadas de alto. Zapato bailarina es preferido. b) Chanclas, sandalias, plataformas, y zapatos de gimnasio no son permitidos. Agujetas deben mantenerse dentro del zapato y amarrados en todo momento.

7 Pelo- Color del pelo debe ser de color natural humano. a) Todo tipo de diseños no son permitidos en el pelo o en la cara. Esto incluye la prohibición de cualquier peinado que distraen o que no sean profesionales. b) No se permiten Mohawks, salmonetes, corte V, o cortes diseñados en el pelo. c) El pelo debe estar fuera de la cara del estudiante, con una apariencia profesional. d) No se permite que las niñas se afeiten los lados de su pelo. Joyería y Accesorios de Cabeza a) Niños no pueden usar aretes y las niñas pueden usar aretes solamente en los oídos. b) Las niñas pueden tener sólo un arete por oído. Pendientes no deben ser más grande que una moneda de 25 centavos en cualquier dirección. c) No se permiten perforaciones visibles, perforaciones en la lengua, o curitas cubriendo perforaciones. d) Todos los collares deben meterse dentro de la camisa. e) Los calcetines deben ser de color negro sólido, blanco, gris, o azul marino. Diseños no están permitidos en los calcetines. f) Accesorios de cabeza no deben ser más grande de 2" en cualquier dirección. g) Una pulsera o reloj en cada mano. No se permiten ligas de goma con diseños o texto. Uniformes de gimnasio Estarán a la venta por $8 y pantalones cortos a $15. Favor de recordarse que estudiantes perderán puntos en la clase de gimnasio si no usan el uniforme. Candados- Cada estudiante debe comprar un candado y traerlo en el primer día de clases. Después que el estudiante obtiene su horario de clases, él/ella debe traer su candado con ellos todo los días que tienen gimnasio. Ejemplo del candado correcto esta abajo. **Como siempre, si no está seguro de qué comprar, por favor llámenos antes de comprarlo. Si tiene alguna duda, guarde su recibo para que pueda devolver los artículos que no se ajusten a las normas. Estaremos encantados de ayudarle a averiguar qué comprar para que su hijo / hija está en uniforme. Información Adicional: Aunque no se requiere, si recomendamos que el estudiante use una mochila duradera y una caja para Lápices/plumas para mantener sus útiles bien organizados. Ciertos maestros requieren otros útiles y será entregada durante orientación. Nos aseguraremos de que materiales adicionales sean mínimos. Lectura requerida del verano: Sobre la terminación de su prueba de la colocación, su niño recibió el libro requerido para la lectura del verano y tarea de escritura.
