Son una serie de estudios que nos permiten determinar la probabilidad de que un feto (bebé que aún no ha nacido) tenga una enfermedad o problema de salud determinados. La probabilidad puede ser alta o baja. Es distinta a un examen diagnóstico que permite determinar si el feto tiene realmente esa enfermedad. Las pruebas de detección más comunes para defectos de nacimiento sirven para descartar defectos del tubo neural (como espina bífida), síndrome de Down o trisomía (triple cromosoma) 18 ó 13. Si su evaluación indica un alto grado de riesgo de uno de estos problemas, usted puede pedir que se le realice un examen diagnóstico. What is screening? A screening test tells us the chances that a fetus (unborn baby) has a certain disease or condition. There may be a high risk or a low risk. This is different from a diagnostic test, which tells us if a fetus actually has that condition. For birth defects, the most common screening tests are for neural tube defects (like spina bifida), Down syndrome, trisomy 18 and trisomy 13. First trimester screening estimates the risk for Down syndrome, trisomy 18 and trisomy 13. It does not tell us the risk for other birth defects or mental retardation. This test requires: A blood test. This measures two chemicals in your blood, called hcg and PAPP-A. These chemicals are made by the pregnancy. An ultrasound. This will measure the amount of fluid at the back of the fetus s neck (an area called the nuchal translucency). Your age. All of this will help us estimate the risk for certain problems. Down syndrome is more likely with higher levels of hcg and lower levels of PAPP-A. Trisomy 18 and trisomy 13 are more likely with lower levels of both hcg and PAPP-A. If there is a large amount of fluid at the back of the neck, there may be a greater risk for Down syndrome, trisomy 18, trisomy 13, heart problems or other conditions. We can add another blood test later to get a better estimate of your risk. This is called a modified screen. We would be happy to discuss this with you. 2 Spanish English 2

All women can have first trimester screening during the early part of their pregnancy. This test is usually done between the start of the 11th week and the end of the 13th week. Before you decide to have this test, talk to your care provider or genetic counselor. He or she will explain the details and answer your questions. What happens during the test? You will meet with a genetic counselor at Fairview Maternal-Fetal Medicine Center. He or she will explain the testing in detail. You will need to sign a consent form. This states that you know the risks and benefits of the screening tests. When you sign the form, you tell us it is okay to do the tests. Do not sign unless you understand what will happen during and after the tests. We will do a blood test and an ultrasound. Results from the blood test will be ready in about seven days. How well does the test work? Most tests show a low risk for Down syndrome, trisomy 18 and trisomy 13. Even if the risk is low, there is still a small chance that your baby will have problems. 3 Spanish English 3

This does not mean that a baby will have problems. It means that more tests are needed. Your genetic counselor will talk with you about the risks and benefits of further testing. Of fetuses that have Down syndrome, trisomy 18 or trisomy 13, most will show an increased risk on this test. But the test is not perfect. Some fetuses with these conditions will have a normal test result. Should I consider second trimester screening as well? Yes. First trimester screening does not tell you the risk for spina bifida and other birth defects that involve an opening on the fetus s skin. To screen for these problems, you may wish to have MSAFP screening. The MSAFP test is a blood test. It is usually done at 15 to 20 weeks of pregnancy. The test measures the amount of a certain protein (called AFP) in your blood. High levels of AFP often suggest a greater risk for certain birth defects or pregnancy problems. At around 18 weeks or later, your care provider may also use an ultrasound to screen for birth defects or other problems. Before you decide to have these tests, talk to your care provider or genetic counselor. He or she will explain the details and answer your questions.
