2 Updating Registration Information Home Access Center (HAC) offers two ways for you to provide ETHS with updated information: Registration > Demographic for simple phone number and/or address changes Registration > Update Registration for more complex changes, including address changes, phone number and/or address changes for one of your student s parents/guardians other than yourself, addition of a parent/guardian, and Emergency Contact changes or additions. If you want to add an address for more than one parent/guardian, you ll need to add that person as an additional guardian. You ll use Registration > Update Registration for that. Simple Phone Number or Address Change You can change your own phone number(s) and address any time via Home Access Center. You can also change your student s phone number(s) and address any time. Here s the easiest way to do that: 1. Log in to Home Access Center using your own parent/guardian user name and password. * 2. 3 3. You will be able to change your own phone number(s) and address here, as well as those of your student. 4. Click Save when you have made your changes, and they will be changed immediately in the school s student information system. If you look in HAC again, you ll see the updated phone number(s) and address(es). You will not be able to change the contact information for other guardians, nor for the Emergency Contact person, from the Registration > Demographic page. For that, you ll need to use the Update Registration page. Page 3 of 20

4 More Complex Updates For more complex updates, you ll need to use the Registration > Update Registration approach. Using this method, you will be able to make changes to additional Registration items: student ethnicity/race student address, phone number and/or address for one of your student s parents/guardians other than yourself Emergency contact additional guardian and/or Emergency contacts Here are the necessary steps: 1. Log in to Home Access Center using your own parent/guardian user name and password. 2. Navigate to Registration > Update Registration, and click the New link. 3. Open the section of the page for the kind of changes you want to make, by clicking the plus sign next to the name of the section. You may also find it helpful to close sections by clicking the minus sign next to the section name after you Save your changes in that section. 5 4. In the Student Information section of this page, you can make changes to the student s Hispanic/Latino Ethnicity, Race, and Address. Be sure to click Save at the bottom of the Student Information section when you finish making changes in the section. Changes to student name, gender, birth date, or language require in-person conversation and documentation for the Registrar s Office. Page 5 of 20

6 5. In the Student Address and Phone section of this page, you can make changes to the student s Physical Address, Mailing Address (if it s a PO Box), and Phone. About addresses: If you change the Physical or Mailing Address, you will need to provide proof of address. You can do that by Uploading a scanned copy in the Documents section at the bottom of this form, mailing it to ETHS, Residency Office, 1600 Dodge Ave, Evanston, IL 60201, or bringing it to the Residency Office in room S124. About phone numbers: Please verify that the Phone Type is correct for any phone number shown on this screen, and update if necessary. To add an additional phone number, click the Add Phone button, select the Phone Type, and type the number in the space provided. To delete a phone number, click to mark the Delete checkbox to the right of the phone number and then click the Delete Phone button. For all changes: Be sure to click Save at the bottom of the Student Address and Phone section when you finish making changes in the section. Page 6 of 20

7 6. In the Parent/Guardian Contact section of this page, you can make changes for any Guardian or Emergency contact. Changes you can make include the Name, Relationship, Language, Address, Education Level, Physical Address, Mailing Address (if it s a PO Box), Phone Numbers. You can also add a contact and change the priority of contacts. a. To make changes for a particular contact, select the Edit/View icon in the row for that contact person. Then scroll down to see existing data and make changes for that contact person. About addresses: If you change the Physical or Mailing Address, you will need to provide proof of address. 8 About Contact Types: Select only one Contact Type per contact, either Guardian or Emergency. Contact Type of Emergency is for an additional contact, someone who is not a parent or guardian of the student. (Do not select Other as the Contact Type.) Page 8 of 20

9 b. To add a Guardian or Emergency contact, click Add contact and then add the relevant information. Be sure to provide first name, last name, and either a phone number or address for each additional Guardian or Emergency contact and to check the box for Guardian or Emergency (choose one or the other not both for the same person). 10 c. If you want to change the order of Priority of the Guardians and/or Emergency contacts, use the arrows in the Adjust Priority column. For all changes: Be sure to click Save at the bottom of the Parent/Guardian Contact section when you finish making changes in the section. 7. If you make an address change for the Student or primary Guardian, you must provide proof of address. You can do that by uploading a scanned copy in the Documents section at the bottom of this form, mailing it to ETHS, Residency Office, 1600 Dodge Ave, Evanston, IL 60201, or bringing to the Residency Office in room S124. 11 8. If you have no other changes to make, click the checkbox next to the I Agree statement at the bottom of the page and then click Submit. 9. After you submit the form, the ETHS Residency and Registrar s Offices will have to review and accept your changes before they will officially be entered into the school s student information system. You will see the changes in Home Access Center only after they have been accepted by the Registrar s Office. Page 11 of 20

14 Actualizaciones Más Complejas Para actualizaciones más complejas necesitará usar Matrícula > Actualización de Matrícula ( Registration> Update Registration ). Usando este método, usted podrá hacer cambios en otros puntos acerca de la Matrícula: Étnia/raza del estudiante Domicilio del estudiante Número de teléfono y/o correo electrónico para uno de los padres/tutores del estudiante aparte de usted Contacto de Emergencia Tutor adicional y/o contactos de Emergencia Estos son los pasos necesarios: 1. Ingrese a Home Access Center usando su propio nombre de usario y contraseña de padre/tutor. 2. Diríjase a Matrícula > Actualización de Matrícula y presione el enlace New ( Nuevo ). Page 14 of 20

15 3. Abra la sección de la página para el tipo de cambios que usted quiere hacer, presionando el signo de adición. 4. En la sección Información del Estudiante ( Student Information ) de esta página, usted puede hacer cambios en la Etnia Latina/Hispana, Raza y Correo Electrónico del estudiante. En la sección de Domicilio del Estudiante y Teléfono ( Student Address and Phone ) de esta página, usted puede hacer cambio al Domicilio Físico, Domicilio Postal (si este es un Casillero Postal P.O. Box ), y teléfono. Acerca del domicilio: Si usted cambia el Domicilio Físico o Domicilio Postal, usted necesitará mostrar prueba de domicilio. Usted puede hacer cargando una copia escaneada en la sección Documentos ubicada en la parte inferior de este formulario, enviarlo a ETHS, Oficina de Domicilio, 1600 Dodge Ave. Evanston, IL 60201, o traerlo a la Oficina de Residencia localizada en el salón S124. 17 6. En la sección de Contacto del Padre/Tutor, usted puede hacer cambios para cualquier contacto del tutor o contacto de emergencia. Los cambios que puede hacer incluyen Nombre, Relación, Idioma, Correo Electrónico, Nivel de Educación, Domicilio Físico, Domicilio Postal (si es que es un Casillero Postal P.O. Box -), Números de Teléfono. Usted también puede añadir y cambiar la prioridad de sus contactos. a. Para hacer cambios de un contacto en particular, elija el ícono de Editar/Ver ( Edit/View ) en la línea del contacto de esa persona. Luego, desplácese hacia abajo para ver la información existente y hacer los cambios de contacto de esa persona. Acerca de los domicilios: Si usted cambia el domicilio físico o postal, usted necesitará mostrar prueba de domicilio. Usted puede hacerlo cargando una copia escaneada en la sección Documentos en la parte inferior de este formulario, enviándolo a la Oficina de Domicilio de ETHS, 1600 Dodge Ave. Evanston 60201, o trayéndolo a la Oficina de Domicilio ubicada en el Salón S124. Acerca de los números de teléfono: Por favor verifique si el Tipo de Teléfono es correcto en cualquier número telefónico mostrado en esta pantalla, y actualízelo si es necesario. Para añadir un nuevo número de teléfono, presione la tecla de Añada Teléfono ( Add Phone ), seleccione el Tipo de Teléfono y escriba el número en el espacio en blanco. Para borrar un número telefónico, presione para marcar la caja de Borrar ( Delete ) en la parte derecha donde está el número de teléfono y luego presione la tecla Borre Teléfono ( Delete Phone ). 18 Acerca de los Tipos de Contacto: Seleccione solamente un Tipo de Contacto, ya sea Tutor o Emergencia ( Guardian or Emergency ). El Tipo de Contacto de Emergencia es para un contacto adicional, alguien que no es una padre o tutor del estudiante. (No seleccione Otro ( Other ) como Tipo de Contacto). Page 18 of 20

19 b. Para añadir un Tutor o Contacto de Emergencia, presione Añada Contacto ( Add contact ) y luego añada la información relevante. Asegúrese de brindar el primer nombre, apellido y, ya sea un número de teléfono o correo electrónico por cada Tutor o contacto de Emergencia adicional, y marcar la caja para Tutor o Emergencia (escoja uno u otro, no ambos para la misma persona). 20 c. Si usted quiere cambiar el orden de Prioridad de los Tutores y/o contactos de Emergencia, use las flechas en la columna de Modificar Prioridad ( Adjust Priority ). Para todos los cambios: Asegúrese de presionar Guardar ( Save ) en la parte inferior de la sección Contacto de Padre/Tutor ( Parent/Tutor Contact ) cuando termine de hacer los cambios en la sección. 7. Si usted hace un cambio de domicilio para el estudiante o Tutor principal, usted debe brindar prueba de domicilio. Usted puede hacer cargando una copia escaneada en la sección Documentos en la parte inferior de este formulario, enviándolo a la Oficina de Domicilio de ETHS, ( ETHS Residency Office ), 1600 Dodge Ave. Evanston, IL 60201, o trayéndola a la Oficina de Domicilio en el salón S Si no tiene otros cambios que hacer, presione la caja ubicada junto a la declaración de Estoy de Acuerdo ( I Agree ) en la parte inferior de la página y luego presione Presentar ( Submit ). 9. Después de presentar el formulario, las Oficinas de Domicilio y Matrícula de ETHS tendrán que revisar y aceptar sus cambios antes de que sean ingresados oficialmente al sistema escolar de información del estudiante. Usted verá los cambios en Home Access Center solo después de que hayan sido aceptados por la Oficina de Matrícula. Page 20 of 20
