AN IMPORTANT MESSAGE REGARDING RELEASE OF STUDENT INFORMATION Please see the back page of this booklet that you can use to communicate to the school your wishes about the release of directory information and education code notifications. If you do not want the Oxnard Union High School District to disclose this information without your consent, please fill out the form on the back page of this handbook and return it to us. If we do not receive the signed form, we will release student information to the military and college recruiters upon their request. If your child is over 18, he or she must sign the form. We can accept these forms anytime, but please know that until your Opt Out form is on file, the information is released. A list of the guidelines on military recruiters and colleges access to information can be found at the following Web site: www.ed.gov/offices/om/fpco. If you have any questions, please do not hesitate to call. Additionally, if you do not want to have your student s standardized test scores appear on the transcript then please turn in the back page of this handbook indicating your choice. Lastly, our schools need to notify parents about school picture dates and various Senior Activities. We would like to give our student addresses to the three vendors that work closely with our schools: Visual Photography, Herff-Jones and Jostens. These are the ONLY vendors that we work with and release addresses to. If you do not want us to share your student s address with these vendors, please let us know your wishes regarding this by turning in the form in the back of this handbook. We want all parents be informed and understand the district s policies. Ask for additional information whenever you have a question, and we will answer them as quickly and efficiently as possible. Our aim is to provide a top educational opportunity for your student. Kathy Greaves Director of Student Services and Interventions Oxnard Union High School District
