Akins High School Band Emergency Treatment Release (Parent Release Form) Dear Parents/Guardians, The following is an authorization form for an Akins High School Band Director employed by the Austin ISD to treat and/or release information regarding injury to a student during a school affiliated music event. I, authorize the Akins High School Band Staff, or their affiliated, associated physicians, or appropriate allied health professionals to evaluate and treat. Additionally, the above named entities may release, exchange, and request pertinent medical and/or examination information concerning or related to this and/or other injuries to authorized and/or an appropriate allied health personnel. All medical information is strictly confidential and will be used only to assist in the evaluation, treatment, or rehabilitation of the injury. Parent/Guardian Signature Date The above named student has my permission to participate in school-sanctioned activities as a member of the Akins High School Band Program during the school year. All transportation will be arranged by the Director, in accordance with AISD guidelines. Students, even when off campus, are still subject to the school rules and regulations when participating with the Akins High School Band Program. I understand that any student who doesn not conduct himself/herself properly may be (1) sent home at the parent s expense; (2) prohibited from participating in future activities of this organization; and/or, (3) subject to other appropriate disciplinary action. Akins Eagle Band Boosters 2015-2016 Program Introduction & Overview Akins Band Families, I want to take the time to introduce the Akins Band Booster Board for the 2015-2016 School year, and to thank you for the trust you have put in me allowing me to serve as the Booster Board Co-President. It is truly an honor and a privilege. I am excited about the upcoming school year. Very simply put, the boosters are a group of parents like yourselves, with varied skills, who support the overall band program. We do that in many ways. We feed, clothe and water the young eagles, through tailgating, uniform maintenance and the Water the Eagles Program. We protect them to keep them safe when they are away from the nest with chaperones at all off campus events. We also do fundraising, and organize volunteers in coordination with the Band Directors. The Boosters work very closely with the teaching staff communicating both to the staff on behalf of parent concerns, and passing important program information from the band directors. Most importantly, the Boosters are a part of the band. We are the support backbone of the Akins Eagle band program. We have many areas where we need your help. It is not necessary for you to commit a lot of your time, but if each parent can commit to just a few hours over the course of the season, we will be able to accomplish so much more to support the overall program. Booster Membership dues are an important part of supporting the band program. Regular members have the privilege of making motions, voting, and holding office. Proceeds from the tiered membership drive (see booster contribution form for details) will be used to fund Scholarships for Akins Seniors and need based scholarships. Our membership drive will kick off on 1 August 2015. We will host a new parent Orientation for Freshman parents, and new parents to Akins at 10-11am on 1 August. Directly following that meeting from 11am-12pm, we will have a meeting for veteran band parents and new parents alike. We will discuss Fundraising and volunteer opportunities, the upcoming show, and support requirements. After the meetings we will have a social time, with games and food for the whole family. Donations will be accepted, both cash and food items. All fundraising sign up opportunities are posted on the Sign-up Genius. La respuesta es muy simple. Los boosters somos un grupo de padres, como ustedes, con talentos/abilidades diferentes que apoyan al programa de la banda. Lo hacemos de muchas maneras. Alimentamos, vestimos, y damos agua a las aguilas de Akins atraves de tailgaiting, mantenimiento de los uniformes, y el programa de Agua para las Aguilas. Los protegemos y los mantenemos seguros cuando se encuentran fuera del nido con los chaperones en todos los eventos que se llevan acabo fuera de la escuela. Tambien recaudamos fondos y organizamos voluntarios en cordinacion con los directores de la banda. Tambien trabajamos en mantener una comunicacion estrecha entre los directores y los padres. Lo mas importante es que los Boosters somos una parte integral de la banda. Somos la via de apoyo de mayor fuerza del programa de la Banda de la escuela Akins. Tenemos muchas areas donde necesitamos su ayuda. No es necesario hacer un compromiso de mucho tiempo. Pero si cada padre de familia se compromete a dedicar solo unas cuantas horas a traves el transcurso de la temporada, como tal, podemos brindar mucho mas apoyo al programa. Fundraising Sign Ups 101 The Akins Band uses the website www.signupgenius.com to manage all concession fundraising opportunities. There is a permanent link to this website on the right hand side of the Akins Band webpage at www.akineseagleband.org. Clicking on this button will take you to a list of all current fundraising opportunities that are available to Akins Band students and families. On this screen, you can see the event and dates available. Clicking on the title of the event will take you to the detailed screen for that event. On this screen, it will show you the date and location of the event. It will also give you general information about the event. There may be one start and end time, or several depending on the event. It will show you the slots that are available to work. Some slots require certain ages, for example 18+ means the worker has to be at least 18, 14+ means the worker has to be at least 14, etc. The first time visiting the site, you will click on the slot for which you wish to sign up, and then click Submit and Sign Up.

The website will ask you to provide your first name, last name, and e mail. Be sure and click the check box that says Create an account at SignUpGenius to save the information and create your account. This will save you a bunch of time in the future. Some sign ups will ask for more information all sign ups will have the field Working for (if not self):. This is where you will put the name of the student to which earned funds go (if it is not yourself). Most sign ups will also ask for your phone number. Please make this your mobile number. There may also be questions asking for t shirt size, or adult names, etc. Please answer all questions. Once you have signed up for the event, you will receive e mail reminders of your upcoming events. In addition, you can swap shifts with others, see your events on the calendar, and there are many other easy ways to manage all the events for which you are earning money. You can always find the list of available sign ups by using the button on the Akins Band webpage at www.akinseagleband.org In the event that technology is unavailable or unfamiliar to you, please don t hesitate to contact a band director or Booster Board member for assistance. How to access parent/student information in Log on to www.charmsoffice.com, and click the ENTER/LOG IN link at upper right. Locate the PARENT/STUDENT/MEMBERS LOGIN section of the web page. Login to your child s program account using the following School Code: AkinsHSBand This will bring up the main parent page. This will allow you to look at the public calendar for your organization, event list, handouts and other files. Clicking on an event on the calendar brings up the details for that event, such as times, attendance requirements, volunteer opportunities, and other event details. Note that if a calendar event is specifically assigned to your child, it may NOT show up on this calendar check the Student calendar after entering the student s ID number (see below). When you enter your child s ID NUMBER (provided by your Director) as a Student Area Password, another more detailed screen appears with even more options to view your student s uniform assignments, music assignments, financial records, forms and inventory. Once you have first entered this ID number, you may create your own, unique password by clicking on the lock (Change Password) icon. Two areas in which you can help the director maintain his/her records: o Update Personal Information if the director has allowed it, you may help make changes to your child s student information page (such as updating phone numbers and email addresses if they change) to help the teacher communicate with you more effectively. Click the Personal Info button. o If your program has setup online payments, you can make credit card payments for fees, trips and deposits to your student s account. If credit card payment is activated, you will see blue buttons in the four main areas of the financial statement indicating your ability to make online payments. You will also see links to enter Practice Logs, view Grades, and use the Recording Studio if the teacher has enabled these options.
