2 So we re going to begin by discussing some basic information on staging. It is defined as the process that determines location and extent of primary cancer at the time of diagnosis. For this session, we will obviously be focusing on breast cancer, although staging does apply to multiple malignancies. So staging includes clinical information which consists of mammogram, ultrasound, pathology, and physical examination findings. So a combination of these two components is what will comprise your staging. The purpose is to guide treatment, is to provide prognostic value. It s important to determine the location and the extent of disease which identifies risk recurrence and mortality data. And that s vital to treatment planning and clinical outcomes. Physicians do consider a patient s stage of disease for clinical trial enrollment. The clinical trials will provide data that allows tumor registrars to report outcomes that will increase our knowledge of cancer biology and research. The AJCC is an organization of health care professionals that was founded in Their purpose was to develop a common nomenclature regarding staging. A uniform system that was able to be used worldwide was then created, known at the TNM staging system. There are various types of staging. So the first type we re going to discuss is the clinical stage, which is pre-treatment or neoadjuvant stage. The physical examination, clinical breast exam, imaging, and diag -- - diagnostics, and laboratory studies will determine the data that compromises the stage of the patient. So things like mammogram, ultrasound of the breast, and nodal basins provides us with the information about the tumor, about its size and location, whether you have a single lesion or multicentric disease, and the presence of absence of suspicious lymph nodes. A biopsy will confirm any disease that is suspected. Additionally, CT of the chest, abdomen, bone scan are also performed to complete the staging, to determine the presence or absence of distant metastatic disease. The pathological stage, post-surgical stage, is --- compiles all clinical staging that you obtained in the pre-treatment phase, plus the data obtained by surgical intervention and pathologic analysis of the tumor and nodal basins. 3 the extent of disease if and when the patient has recurrence of their primary tumor. The most common sites in breast cancer are the brain, bone, liver, lung, and chest wall. Also performed on patient --- this is also performed on patients that [are] being treated for metastatic disease to assess the response to their current therapies. Again, CT of the chest, abdomen, bone scan, or a PET scan as well as laboratory studies and tumor markers, CA 15-3 and CA 27.29, which are specific to breast tumors, is what is utilized for restaging. The TNM staging system consists of standard algorithms of disease by anatomic site and histology. The T stands for tumor size. We assign values of 1 to 4. The N is for nodal involvement, assigned values 1 to 3. And the M is for metastatic disease to distant organs, given the numbers 0 to 1. The higher the value, the greater extent of disease, and the worse prognosis for the patient. Subgroups provide more specific information on disease. For example, a --- a T1 lesion measures less than 2 cm. And if you break that down into subgroups, a T1a will measure greater than 0.1 cm but less than or equal to 0.5 cm. A T1b will be a measurement of greater than 0.5 cm but less than or equal to 1 cm, and so on. This, as I said, will further break down the size of the lesion. Each T, N, and M number value is combined into one stage group, given the Roman numeral value 0 through IV. primario. Assessment of the nodal status can be determined by several modalities. Of course, as we just discussed, ultrasound is one way. Another way is excisional biopsy of a suspicious node and that will be performed in the neoadjuvant setting for clinical confirmation of nodal disease. A sentinel lymph node biopsy and/or completion axillary dissection is a more invasive surgical approach that is utilized for locally advanced disease. This will identify the number of node --- nodes involved with disease and help us to further clarify our staging. So Nx is regional lymph nodes that cannot be assessed, such as if the patient comes to MD Anderson for an initial visit and has not had an ultrasound of the nodal basins. N0 is the absence of regional lymph node metastasis. If the patient has one to three lymph nodes involved with disease, they will be classified as an N1.
